Νέες Οδηγίες & Κατευθυντήριες Γραμμές για την Καρδιοπνευμονική Αναζωογόνηση και Επείγουσα Καρδιοαγγειακή Φροντίδα μόλις δημοσιεύθησαν, κατ’αρχήν το Νοέμβριο 2005 στο περιοδικό Circulation (Circulation. 2005;112:Suppl III) και ταυτόχρονα στο περιοδικό Resuscitation (Resuscitation 2005, 67S1), και ακολούθως σε Συμπληρωματικό Τεύχος (Supplement) του Circulation (13 Δεκεμβρίου 2005). Οι Οδηγίες αυτές είναι ελεύθερα διαθέσιμες στο διαδίκτυο (http://www.circulationaha.org/ http://www.C2005.org). Οι πιο σημαντικές αλλαγές περιλαμβάνουν την απάλειψη της εκτίμησης των ζωτικών σημείων από τον απλό διασώστη πριν ν’αρχίσει τις μαλάξεις, δίνοντας οδηγία να δώσει 2 τεχνητές αναπνοές στο θύμα που δεν ανταποκρίνεται και δεν αναπνέει και να ξεκινήσει αμέσως μετά τις μαλάξεις. Επίσης υποδεικνύουν μία γενική αναλογία μαλάξεων/αναπνοών 30:2 για τον ένα διασώστη, και δυνατές & γρήγορες μαλάξεις 100/min. Στους επαγγελματίες διασώστες συνιστούν 5 κύκλους (ή ~2min) βασικής ανάνηψης (CPR) πριν να δώσουν απινίδωση, ιδίως εάν η καθυστέρηση ήταν 4-5 min από την κλήση έως την άφιξη, και τους ίδιους 5 κύκλους μεταξύ ελέγχων ρυθμού. Υποδεικνύουν τη χορήγηση 1 (αντί 3) shock που ακολουθείται αμέσως από CPR. Δίδουν έμφαση στη χορήγηση θρομβόλυσης τόσο στο οξύ έμφραγμα όσο και στο οξύ εγκεφαλικό από ιατρούς μέσω καθορισμένου πρωτοκόλλου. Βεβαίως δίδεται έμφαση στην εφαρμογή προγραμμάτων για τη διάδοση της εκπαίδευσης στην ανάνηψη στο γενικό πληθυσμό και τη διάδοση στη χρήση αυτόματων εξωτερικών απινιδωτών.

Στις 21 Φεβρουαρίου 2006 δημοσιεύθηκαν αναθεωρημένες οι Αμερικανικές οδηγίες εκπαίδευσης στην Καρδιολογία (JACC 2006:894-920) (http://www.acc.org/clinical/competence/nuclear/COCATSfinal.pdf). Σε αυτές περιλαμβάνονται και οδηγίες για την εκπαίδευση στην Ηλεκτροφυσιολογία & Βηματοδότηση επικυρωμένες από τις εταιρείες ACC/AHA/HRS. Διαχωρίζονται 3 επίπεδα εκπαίδευσης: το επίπεδο 1 (Level 1), εντός του προγράμματος της κύριας εκπαίδευσης στην Καρδιολογία, πρέπει να συμπεριλαμβάνει τουλάχιστον 2 μήνες κλινικής καρδιακής Ηλεκτροφυσιολογίας, όπου ο ειδικευόμενος θα πρέπει να μάθει τις ενδείξεις και περιορισμούς των ηλεκτροφυσιολογικών μελετών, φαρμακολογικών και μη-φαρμακολογικών επιλογών, να εκτεθεί στις τεχνικές διάγνωσης και θεραπείας των αρρυθμιών, να μάθει τα βασικά για την καρδιακή βηματοδότηση και απινίδωση και τις ενδείξεις της αμφικοιλιακής βηματοδότησης και να εκτελέσει τουλάχιστον 10 εισαγωγές προσωρινών βηματοδοτών και 10 ηλεκτρικές ανατάξεις. Στο επίπεδο 2 (Level 2) παρέχεται προχωρημένη εκπαίδευση για τουλάχιστον 6 μήνες για να μπορέσει ο εκπαιδευόμενος να εργάζεται στη μη-επεμβατική αρρυθμιολογία και στα εξωτερικά ιατρεία βηματοδοτών και απινιδωτών. Σε αυτό το επίπεδο αναμένεται ο υποψήφιος ανεξάρτητα να διενεργήσει προγραμματισμό σε τουλάχιστον 100 ασθενείς με ηλεκτρικές συσκευές, αλλά δεν δύναται να εκτελέσει ακόμη επεμβάσεις. Στο επίπεδο 3 (Level 3) που διαρκεί τουλάχιστον 1 έτος πέραν από τα 3 βασικά, δηλαδή συνολικά 4 έτη εκπαίδευσης, αλλά συνιστάται και πρακτικά απαιτείται και πρόσθετο (5ο) έτος με συνολική πρόσθετη διετή αφοσιωμένη εκπαίδευση στην Ηλεκτροφυσιολογία & Βηματοδότηση. Στο επίπεδο 3 ο εκπαιδευόμενος πρέπει να εκτελέσει 150 ηλεκτροφυσιολογικές επεμβάσεις και να είναι ο κύριος χειριστής και αναλυτής 100-150 αρχικών διαγνωστικών εξετάσεων. Τουλάχιστον οι 50-75 πρέπει να αφορούν υπερκοιλιακές αρρυθμίες. Πρέπει να είναι ο κύριος χειριστής για τουλάχιστον 25 ηλεκτροφυσιολογικές αξιολογήσεις εμφυτευόμενων αντιαρρυθμικών συσκευών. Εμπειρία πρέπει επίσης να αποκτηθεί τουλάχιστον σε 10 transseptal καθετηριασμούς και 75 καταλύσεις αρρυθμιών... (excerpt)

Υπάρχει σαφής, αυξανόμενη και αδήριτη ανάγκη και απαίτηση στην κοινωνία για κατάδειξη ποιότητας στην παρεχόμενη ιατρική φροντίδα σε κάθε σύστημα υγείας. Ένα μέτρο προς αυτή την κατεύθυνση είναι η δημοσίευση των αποτελεσμάτων των καρδιακών επεμβάσεων και παρεμβατικών πράξεων, μέτρο που έχει υιοθετηθεί στις ΗΠΑ σε αρκετές πολιτείες. Μάλιστα στη Νέα Υόρκη, παρά την αρχική κατακραυγή για το μέτρο, αυτό έχει οδηγήσει σαφώς σε μείωση της θνητότητας στις καρδιακές παρεμβάσεις. Εναλλακτικά έχει προταθεί η εθελοντική συνεργασία νοσηλευτικών ιδρυμάτων στην καταγραφή και τον έλεγχο της έκβασης των καρδιολογικών πράξεων και κυρίως των θανάτων και τη λήψη κατάλληλων μέτρων αύξησης της συμμόρφωσης και συνεργασίας με αποτέλεσμα την περαιτέρω μείωση της θνητότητας και τη βελτίωση της ποιότητας της παρεχόμενης καρδιακής φροντίδας. Δυστυχώς στη Χώρα μας πόρω απέχουμε από μια τέτοια εφαρμογή αυστηρών μέτρων επιτήρησης και ποιοτικού ελέγχου είτε από την πολιτεία είτε αυτοελέγχου από τα ίδια τα νοσηλευτικά ιδρύματα και τους γιατρούς. Είναι όμως πιθανόν ένας τέτοιος έλεγχος να επιβληθεί απο άλλους φορείς που θεωρούν ότι είναι πλέον δικαίωμα κάθε ασθενούς και του κοινωνικού συνόλου γενικότερα να γνωρίζει το αποτέλεσμα και την έκβαση σε συγκεκριμένες επεμβάσεις σε κάθε νοσηλευτικό ίδρυμα και σε κάθε γιατρό έτσι ώστε να είναι σε θέση να επιλέξει που θα εμπιστευθεί την υγεία του και τη ζωή του! Τον Σεπτέμβριο 2006 δημοσιεύθησαν οι κοινές Κατευθυντήριες Οδηγίες των ACC/AHA/ESC για την αντιμετώπιση των κοιλιακών αρρυθμιών και του αιφνιδίου καρδιακού θανάτου. Σε ασθενείς με στεφανιαία νόσο οι ενδείξεις ηλεκτροφυσιολογικής μελέτης (ΗΦΜ) περιλαμβάνουν: ασθενείς με παλαιό έμφραγμα και ύποπτα συμπτώματα κοιλιακών ταχυαρρυθμιών (ταχυπαλμία, προσυγκοπή & συγκοπή), ως καθοδήγηση κατάλυσης κοιλιακής ταχυκαρδίας (ΚΤ), για τη διερεύνηση ταχυκαρδίας με ευρέα QRS (όλα class I), & για διαστρωμάτωση κινδύνου σε ασθενείς με παλαιό έμφραγμα, μη-εμμένουσα ΚΤ και κλάσμα εξώθησης <40% (class IIa). Στη διατατική μυοκαρδιοπάθεια ΗΦΜ ενδείκνυται για τη διάγνωση & κατάλυση ΚΤ επανεισόδου των σκελών, σε ασθενείς με ταχυπαλμία, ταχυκαρδία με ευρέα QRS, προσυγκοπή ή συγκοπή (class I)... (excerpt)

Όλοι συμφωνούν ότι η διαστρωμάτωση κινδύνου για τον αιφνίδιο καρδιακό θάνατο (ΑΚΘ) πάσχει και οι τρέχουσες μέθοδοι είναι ανεπαρκείς, ωστόσο οι νέες οδηγίες δεν αντανακλούν αυτόν τον προβληματισμό και συστήνουν εμφύτευση απινιδωτή για πρωτογενή πρόληψη με βάση το κλάσμα εξώθησης (ΚΕ). Πράγματι το ΚΕ σαν συνεχής μεταβλητή σχετίζεται με αυξημένο κίνδυνο, όσο χαμηλότερο τόσο μεγαλύτερη η θνητότητα, αλλά είναι μόνον μια από τις πολλές μεταβλητές και ένας μόνον από τους πολλούς παράγοντες κινδύνου που σχετίζονται στατιστικά με τη θνητότητα και συνδέονται με το υπόστρωμα και τους πυροδότες. Για να εκτιμήσει κανείς την κλινική χρησιμότητά του, θα πρέπει να εξετάσει την ευαισθησία και την ειδικότητά του, κι εδώ όλοι πάλι συμφωνούν πως το ΚΕ υπολείπεται. Επομένως είναι εμφανείς οι κίνδυνοι να στηρίζεται κανείς μόνον στο ΚΕ, με αποτέλεσμα ασθενείς με ΚΕ μεγαλύτερο από το αυθαίρετο όριο (35% ή 30%) καταλήγουν αιφνιδίως, και ασθενείς με χαμηλότερο ΚΕ που δεν προβλέπει σωστά τον κίνδυνο λαμβάνουν δαπανηρή θεραπεία που δεν χρειάζονται. Ακόμη και στις κλασικές μελέτες η αποτελεσματικότητα του απινιδωτή είναι μικρότερη στις πιο πρόσφατες από ότι στις αρχικές μελέτες. Σύμφωνα με τη μελέτη SCD-HeFT, 25 απινιδωτές πρέπει να εμφυτευθούν για να σωθεί 1 ζωή σε περίοδο 2 ετών. Πιθανότατα η καλύτερη πρόγνωση που έχει επιτευχθεί με τις τρέχουσες φαρμακολογικές θεραπείες για την καρδιακή ανεπάρκεια να ευθύνεται για τη μείωση του οφέλους από τον απινιδωτή. Η χαμηλή προβλεπτική αξία των τρεχουσών μεθόδων στη διαστρωμάτωση του κινδύνου είναι αποτέλεσμα της αποτυχίας των στο να ανακαλύπτουν πλήρως το παθολογικό ηλεκτρικό αποτύπωμα που αυξάνει το ευάλωτο στην ηλεκτρική αστάθεια. Είναι καθ’οδόν κλινικές μελέτες που εξετάζουν εάν η ανάλυση των βιοηλεκτρικών σημάτων πέριξ του αρρυθμογενούς ιστού βελτιώνει τη διαστρωμάτωση του κινδύνου... (excerpt)

Η κλασική καρδιο-πνευμονική αναζωογόνηση /ανάνηψη περιλαμβάνει τις καρδιακές μαλάξεις μαζί με τη(τον) στόμα-με-στόμα αναπνοή (αερισμό) από τους παρευρισκόμενους που θεωρούνται ως ζωτικοί κρίκοι της αλυσίδας επιβίωσης για τα άτομα με καρδιακή ανακοπή. Ωστόσο, πολλοί είναι εκείνοι που διστάζουν στην εποχή μας ή το θεωρούν φραγμό να δώσουν στόμα-με-στόμα αερισμό. Η ομάδα μελέτης SOS-KANTO (περιοχή Kanto της Ιαπωνίας) διενήργησε μια μελέτη παρατήρησης σε θύματα με εξωνοσοκομειακή καρδιακή ανακοπή. Οι προσπάθειες ανάνηψης των παρευρισκομένων αξιολογήθηκαν από ομάδα παραϊατρικών επαγγελματιών κατά την άφιξή των στο σημείο της ανακοπής. Το ποσοστό των ασθενών με ευνοϊκή νευρολογική έκβαση ήταν υψηλότερο σε όσους είχαν οποιαδήποτε ανάνηψη σε σύγκριση με όσους δεν είχαν καθόλου ανάνηψη. Η καρδιακή-μόνον ανάνηψη είχε μεγαλύτερα ποσοστά ευνοϊκού νευρολογικού αποτελέσματος στις υπο-ομάδες με άπνοια, με ρυθμό που μπορούσε να απινιδωθεί, ή είχε λάβει ανάνηψη εντός 4 λεπτών από την ανακοπή. Η συμβατική (κλασική) ανάνηψη δεν παρέσχε κανένα πρόσθετο όφελος για καμία υπο-ομάδα. Συγκεκριμένα, 439 (11%) από 4068 ενήλικα άτομα με εξωνοσοκομειακή καρδιακή ανακοπή έλαβαν μόνον καρδιακές μαλάξεις από τους παρευρισκόμενους, 712 (18%) έλαβαν κλασική ανάνηψη με καρδιοαναπνευστική αναζωογόνηση, και 2917 (72%) δεν έλαβαν καθόλου επιτόπια ανάνηψη πριν την έλευση των παραϊατρικών επαγγελματιών. Παραμένει Δεύτερης Επιλογής Ένδειξη (Class 2A/ Επίπεδο Μαρτυρίας C) η Κατάλυση της Κολπικής Μαρμαρυγής και Μόνον σε Συμπτωματικούς Ασθενείς Ανθεκτικούς στα Φάρμακα & με Πολλούς Περιορισμούς

Σύμφωνα με τη νέα δήλωση των εταιρειών HRS/EHRA/ECAS/ACC/AHA/STS (HR 2007;4 (6):816-861), υιοθετείται η ένδειξη των Οδηγιών ACC/AHA/ESC του 2006 για την κύρια ένδειξη κατάλυσης της κολπικής μαρμαρυγής (κΜ), δηλ. σαν εναλλακτική λύση της φαρμακοθεραπείας σε συμπτωματικούς ασθενείς με μικρή ή καθόλου διάταση του αριστερού κόλπου (ένδειξη 2Α, μαρτυρία C). Τονίζεται ότι θα πρέπει να προηγείται αγωγή των προκαλούντων ή αναστρέψιμων αιτίων της κΜ πριν από την έναρξη της φαρμακοθεραπείας. Η κατάλυση ορίζεται ως δέυτερης γραμμής θεραπεία για όλες τις κατηγορίες ασθενών. Είναι αποδεκτή μόνον επι παρουσίας συμπτωματικής κΜ ανθεκτικής ή μη-ανεκτής σε 1 τουλάχιστον κατηγορίας 1 ή 3 αντιαρρυθμικό φάρμακο. Η παρουσία θρόμβου στον αριστερό κόλπο είναι απόλυτη αντένδειξη για την κατάλυση. Λόγω της απαιτητικής τεχνικής και των σοβαρών επιπλοκών που ενέχει η επέμβαση, οι κίνδυνοι και τα οφέλη θα πρέπει να ζυγισθούν πολύ προσεκτικά. Ο υψηλός κίνδυνος διάτρησης του μυοκαρδίου στους υπερήλικες και οι θρομβοεμβολικές επιπλοκές με την χαμηλότερη πιθανότητα επιτυχίας επι μεγάλης διάτασης του αριστερού κόλπου, πρέπει να λαμβάνονται σοβαρά υπ’όψη. Επίσης η επιτυχία είναι πολύ μικρότερη σε εμμένουσα κΜ μεγάλης χρονικής διάρκειας... (excerpt)

Δεξιά Διεστιακή Κοιλιακή Βηματοδότηση: Ίσως μια Εναλλακτική Λύση για τις Ανέφικτες ή Δύσκολες Περιπτώσεις Αμφικοιλιακής Βηματοδότησης?

Σύμφωνα με τη μελέτη BRIGHT σε 42 ασθενείς με τυπικές ενδείξεις για αμφικοιλιακή βηματοδότηση, έγινε τυχαιοποίηση μετά τον πρώτο μήνα σε διεστιακή δεξιά κοιλιακή βηματοδότηση ή control mode (VVI @ 40 ppm). Η διεστιακή βηματοδότηση περιέλαβε την κορυφή και υψηλή διαφραγματική θέση της δεξιάς κοιλίας. Η μελέτη ήταν διασταυρούμενη 6μηνης διάρκειας και η διασταύρωση έλαβε χώρα στο 3μηνο. Οι παράμετροι μετρήθηκαν πριν την τυχαιοποίηση και μετά 3 μήνες διεστιακής ή control mode βηματοδότησης. Οκτώ ασθενείς δεν είχαν 7μηνη παρακολούθηση, 3 απεβίωσαν (1 πριν την τυχαιοποίηση), & 8 εξήλθαν της μελέτης. Σε 1 ασθενή αντικαταστάθηκε το ηλεκτρόδιο με αριστερό κοιλιακό ηλεκτρόδιο και 1 ασθενής εμφάνισε κολπική μαρμαρυγή. Σε σύγκριση με τις βασικές μετρήσεις, η διεστιακή βηματοδότηση βελτίωσε το κλάσμα εξώθησης από 26+12% σε 36+11% (p<0.0008), & η κατηγορία κατά ΝΥΗΑ μειώθηκε από 2.8+0.4 σε 2.3+0.7 (P < 0.007). Επιπλέον, η 6λεπτη βάδιση βελτιώθηκε από 372+129 m σε 453+122 m (P < 0.05), και η ποιότητα ζωής βελτιώθηκε σημαντικά (P < 0.006). Στην ομάδα ελέγχου δεν παρατηρήθηκαν σημαντικές μεταβολές. Προβληματική η Αξιοπιστία των Απινιδωτικών Ηλεκτροδίων: Ανακλήσεις & Αναφορές Επιπλοκών & Δυσλειτουργίας

Το πρόβλημα με τα απινιδωτικά ηλεκτρόδια έγινε έντονο τον τελευταίο καιρό λόγω της πρόσφατης ανάκλησης των ηλεκτροδίων τύπου Sprint Fidelis (βλ. Προηγούμενο τεύχος: ΡΥΘΜΟΣ 2007, 2(8):58 ) και των συνεχιζόμενων αναφορών προβλημάτων με τα ηλεκτρόδια τύπου Riata (St. Jude). Συγκεκριμένα σε αναφορά απο τη Γερμανία το Μάϊο 2007 σε 990 διαδοχικούς ασθενείς, εμφανίστηκε δυσλειτουργία 148 (15%) απινιδωτικών ηλεκτροδίων (>20 μοντέλα και των 5 εταιρειών: CPI/Guidant, Medtronic, St. Jude, ELA, & Biotronik). Παρατηρήθηκε προοδευτική ετήσια αύξηση στη δυσλειτουργία των ηλεκτροδίων και στα 10-ετή ηλεκτρόδια το 20% είχε προβλήματα. Τα κυριότερα προβλήματα αφορούσαν ρήξη της μόνωσης (56%), ρήξη αγωγού (12%), απώλεια σύλληψης (11%), παθολογική αντίσταση (10%) και απώλεια ανίχνευσης (10%). Οι ασθενείς με τα προβλήματα ηλεκτροδίων ήσαν κατά 3 έτη νεότεροι και περισσότερες γυναίκες. Σε αναφορά απο τις ΗΠΑ για 130 ηλεκτρόδια Riata (μοντέλα 1580/1581), αναφέρθηκαν 5 διατρήσεις καρδιάς (Danik et al, HR 2007;4:439). Μάλιστα οι ίδιοι συγγραφείς στη συνέχεια ανέφεραν οτι σε 115 ηλεκτρόδια Riata, μοντέλα 7000, 7001, 7011, με μικρότερη διάμετρο 6.3F, οι διατρήσεις καρδιάς σε 3 ασθενείς οδήγησαν τους εμφυτευτές να επανέλθουν στο μεγαλύτερης διαμέτρου ηλεκτρόδιο, μοντέλο 1591, με το οποίο δεν είχαν παρατηρήσει καμία διάτρηση (Danik, HR 2007;4:1248)...(excerpt)

Αθροίζονται συνεχώς νεότερα βιβλιογραφικά δεδομένα που επιβεβαιώνουν τη βλαπτική επίδραση της βηματοδότησης στην κορυφή της δεξιάς κοιλίας. Σε μελέτη 304 ασθενών με επίκτητο κ-Κ αποκλεισμό που έλαβαν κλασικό βηματοδότη, ενώ δεν είχαν προηγούμενο ιστορικό καρδιακής ανεπάρκειας, μετά από διάμεση παρακολούθηση ~8 ετών, 79 (26%) ανέπτυξαν νεοεμφανισθείσα καρδιακή ανεπάρκεια. Στην πολυπαραγοντιακή ανάλυση κατεδείχθη η συσχέτιση εμφάνισης καρδιακής ανεπάρκειας με τη μεγαλύτερη ηλικία κατά την εμφύτευση του βηματοδότη (hazard ratio-HR 1.06), με τη στεφανιαία νόσο (HR 1.98), και με το ευρύτερο βηματοδοτικό QRS (HR 1.27). Επιπλέον, η καρδιαγγειακή θνητότητα ήταν σημαντικά μεγαλύτερη σε όσους είχαν καρδιακή ανεπάρκεια μετά απο βηματοδότηση της κορυφής της δεξιάς κοιλίας (36.7% vs. 2.7%, P < 0.001) (Zhang et al, JCE 2008;19:136). Μόλις δημοσιεύθησαν οι νέες οδηγίες και κατευθυντήριες γραμμές για την εμφύτευση βηματοδοτών και απινιδωτών, οι οποίες λαμβάνουν υπ’όψη πολλά απο τα νεότερα δεδομένα που αφορούν στη θεραπεία με συσκευές των διαταραχών του καρδιακού ρυθμού (Epstein et al, Circulation 2008, May 27;e350-408 & JACC 2008;51(21):2085-105 & www.acc.org ή http://my.americanheart.org ή www.hrsonline.org). Για πρώτη φορά γίνεται μνεία στις οδηγίες αυτές για τη σημασία της ελαχιστοποίησης της μη-απαραίτητης κοιλιακής βηματοδότησης λόγω των βλαπτικών επιδράσεων της βηματοδότησης της κορυφής της δεξιάς κοιλίας, για τη χρήση νέων τρόπων βηματοδότησης με αλγόριθμους αποφυγής της κοιλιακής βηματοδότησης πλήν περιόδων υψηλού βαθμού κ-Κ αποκλεισμού και για μελέτες σύγκρισης των επιδράσεων της βηματοδότησης σε εναλλακτικές θέσεις (βλ. άρθρο σύνταξης στο προηγούμενο τεύχος του “Ρυθμού”). Επίσης γίνεται αναφορά στη διατηλεφωνική ή στη μακρόθεν παρακολούθηση των συσκευών, αν και συγκεκριμένες οδηγίες δεν προτείνονται ακόμη για τέτοιου είδους παρακολούθηση. Είναι ενδιαφέρον ότι οι νέες οδηγίες για την εμφύτευση του απινιδωτή (ICD) αναγνωρίζουν τα προβλήματα των προηγούμενων χαλαρών οδηγιών του 2006 για την πρωτογενή πρόληψη που βασίζονταν μόνον στο κλάσμα εξώθησης, και περιορίζουν τις ενδείξεις σε ασθενείς με κλινικό προφίλ παρόμοιο με εκείνο των συγκεκριμένων μελετών και με τα ειδικά κριτήρια αυτών των μελετών... Ο κίνδυνος για αιφνίδιο καρδιακό θάνατο (ΑΚΘ) υφίσταται ως μια συνεχής μεταβλητή, ωστόσο οι προσπάθειες που έχουν γίνει αφορούν στη διχοτομημένη αντίληψη για υψηλό ή χαμηλό κίνδυνο. Μάλιστα η επιδημιολογική πραγματικότητα υποδεικνύει ότι οι περισσότεροι ΑΚΘ παρατηρούνται σε εκείνους τους ασθενείς με παράγοντες χαμηλού ή ενδιάμεσου κινδύνου και σε εκείνους που δεν έχουν γνωστούς παράγοντες κινδύνου. Στην πραγματικότητα οι υπο-ομάδες υψηλότατου κινδύνου, στις οποίες έχει εστιαστεί η μεγαλύτερη προσοχή λόγω του μεγέθους του κινδύνου για ΑΚΘ, αποτελούν μόνον ένα μικρό ποσοστό του συνολικού αριθμού ετησίων θανάτων. Προσδιορίζοντας επομένως τον κίνδυνο με στενά κριτήρια, μπορεί να διακρίνει τα επιλεγμένα εκείνα άτομα στα οποία οι παρεμβάσεις μας θα είναι επωφελείς, αλλά αυτό βοηθάει ελάχιστα την κοινωνία σαν σύνολο. Μεταξύ των προγνωστικών δεικτών που έχουν εξεταστεί, μόνον το χαμηλό κλάσμα εξώθησης (ΚΕ) φαίνεται ότι έχει καταδειχθεί χρήσιμο στην ανεύρεση υψηλού κινδύνου ασθενών στους οποίους η προληπτική θεραπεία (εμφύτευση του αυτόματου απινιδωτή-ICD), θα αποτρέψει αποτελεσματικά τον ΑΚΘ. Ωστόσο, το χαμηλό ΚΕ έχει περιορισμένη ευαισθησία, καθ’όσον οι περισσότεροι ΑΚΘ επέρχονται σε ασθενείς με διατηρημένο ΚΕ. Οι άλλοι δείκτες, όπως τα όψιμα δυναμικά, το εύρος του QRS, η μεταβλητότητα της καρδιακής συχνότητας (HRV), κοιλιακή εκτοπία & μη-εμμένουσα κοιλιακή ταχυκαρδία στο Holter, η ευαισθησία των τασεοϋποδοχέων (BRS) και ο εναλλασσόμενος του Τ (TWA), έχουν περιορισμένη κλινική χρησιμότητα, κυρίως λόγω της χαμηλής θετικής προβλεπτικής αξίας που έχουν... Η ερευνητική ομάδα απο την Tucson, Arizona των ΗΠΑ έχει εφαρμόσει απο το 2003 τη δικιά της προσέγγιση στην καρδιοπνευμονική αναζωογόνηση, την καρδιοεγκεφαλική αναζωογόνηση (CCR), γιατί είχε πεισθεί απο έρευνα δεκαετιών ότι οι τρέχουσες κατευθυντήριες οδηγίες είναι λανθασμένες, και παρατήρησε δραματική βελτίωση στα ποσοστά επιβίωσης των θυμάτων εξωνοσοκομειακής καρδιακής ανακοπής (J Am Coll Cardiol 2009, 53:149). Η πρόταση και η μέθοδος της ομάδος συνίσταται σε 3 στοιχεία: 1) συνεχείς καρδιακές μαλάξεις απο παρευρισκόμενους στην ανακοπή, 2) νέο αλγόριθμο προχωρημένης ανάνηψης (ACLS), & 3) επιθετική φροντίδα μετά την ανάνηψη με θεραπευτική υποθερμία & πρώϊμη παρέμβαση μέσω καθετηριασμού & στεφανιογραφίας. Η καρδιοεγκεφαλική ανάνηψη προτείνει τις συνεχείς καρδιακές μαλάξεις χωρίς τον απο του στόματος αερισμό όπως πλέον και άλλες ομάδες έχουν πρόσφατα προτείνει (βλ. “Ρυθμός” τεύχος 6, Απρίλιος 2007) (http://www.evagelismos.dom.gr/). Για την ACLS, προτείνεται άμεση ή καθυστερημένη απινίδωση ανάλογα με τη φάση της κοιλιακής μαρμαρυγής βασισμένη σε 3φασικό μοντέλο. Για τους παρευρισκόμενους την ώρα της ανακοπής, με πρόσβαση σε αυτόματο εξωτερικό απινιδωτή, και για το προσωπικό του ασθενοφόρου που καταφθάνουν κατά την ηλεκτρική φάση (δηλ. τα πρώτα 4-5 λεπτά της ανακοπής απο κοιλιακή μαρμαρυγή), συνιστάται άμεση απινίδωση... Χαμηλή η Επίπτωση Αιφνιδίου Θανάτου στην Εποχή της Πρωτογενούς Αγγειοπλαστικής

Σύμφωνα με το αρχείο της ιαπωνικής μελέτης HIJAMI ΙΙ (Heart 2009, 95:216), περίπου 78% των 4122 ασθενών με οξύ έμφραγμα που επέζησαν και εξήλθαν απο το νοσοκομείο είχαν υποβληθεί σε πρωτογενή αγγειοπλαστική (περίπου 4% υποβλήθηκαν σε αορτοστεφανιαία παράκαμψη). Σε μέση διαχρονική παρακολούθηση ~4 ετών, η επίπτωση του αιφνιδίου θανάτου ήταν μόνον 1.2% και η συνολική θνητότητα 13.1%. Ασθενείς με χαμηλό κλάσμα εξώθησης (ΚΕ) είχαν μεγαλύτερη επίπτωση αιφνιδίου (Εικόνα) και ολικού θανάτου, αλλά η ανεύρεση χαμηλού ΚΕ περιορίστηκε σε σχετικά χαμηλή συχνότητα: 17.1% των ασθενών είχαν ΚΕ <40% και μόνον 4.8% των ασθενών είχαν ΚΕ <30%. Παρομοίως σε άλλη μελέτη πρωτογενούς αγγειοπλαστικής (Zwolle Myocardial Infarction Study)(Am J Cardiol 2007;100:793), KE <30% παρατηρήθηκε σε ποσοστό 13.4% μεταξύ 2544 ασθενών με STEMI που υποβλήθηκαν σε πρωτογενή αγγειοπλαστική. Στην ίδια μελέτη επήλθαν μόνον 8 αιφνίδιοι θάνατοι (ποσοστό 2.3%) τον πρώτο χρόνο μεταξύ 342 ασθενών που είχαν ΚΕ<30%. Σε άλλη ιαπωνική μελέτη, (Circ J 2005;69:19) παρατηρήθηκαν μόνον 2 αιφνίδιοι θάνατοι σε 37 μήνες μεταξύ 99 ασθενών με έμφραγμα και ΚΕ<30% που είχαν υποβληθεί κατά κύριο λόγο (77%) σε πρωτογενή αγγειοπλαστική (10% είχαν υποβληθεί σε εγχείρηση “bypass”). Τα αποτελέσματα αυτά δίνουν έμφαση στη σημασία που έχει η βέλτιστη επαναγγείωση στη μείωση του αιφνιδίου θανάτου... (excerpt)

Σύμφωνα με αρκετές βιβλιογραφικές αναφορές περιπτώσεων, ποικίλες μέθοδοι φυσιοθεραπείας ενέχουν άλλοτε άλλο κίνδυνο αρνητικών και βλαπτικών αλληλεπιδράσεων με τις εμφυτέυσιμες ηλεκτρονικές καρδιαγγειακές συσκευές (βηματοδότες & απινιδωτές). Η ανησυχία και ο φόβος τέτοιων αλληλεπιδράσεων οδηγεί στην αποφυγή ή μείωση της χρήσης τέτοιων μεθόδων στους καρδιοπαθείς ασθενείς που φέρουν αυτές τις συσκευές. Δυστυχώς ως τώρα δεν υπάρχουν κατευθυντήριες οδηγίες για τη χρήση τέτοιων μεθόδων και οι πληροφορίες που έχουμε στη διάθεσή μας πηγάζουν απο τις δημοσιευμένες περιπτώσεις και απο τις περιορισμένες οδηγίες των κατασκευαστριών εταιρειών. Ο αναγνώστης παραπέμπεται για το θέμα αυτό στην πρόσφατη δημοσίευση των Digby GC et al (Europace 2009, 11:850). Φαίνεται ότι είναι εφικτό να εφαρμοσθούν φυσιοθεραπευτικές μέθοδοι σε ασθενείς με ηλεκτρονικές συσκευές, εάν ληφθούν τα κατάλληλα μέτρα. Οι συγγραφείς παρουσιάζουν μια αναδρομική ανάλυση 25 ασθενών με συσκευές (22 βηματοδότες/3 απινιδωτές) που είχαν 230 επισκέψεις σε εργαστήριο φυσιοθεραπείας. Υποβλήθηκαν σε διαδερμική ηλεκτρική νευροδιέγερση (transcutaneous electrical nerve stimulation-TENS) (ν=5), ή/και θεραπεία με υπέρηχο (ν=15), βελονισμό (ν=19), Laser (ν=7), έλξη/χειροθεραπεία (ν=12), άσκηση (ν=8), εκπαίδευση (ν=18), ταινιοδεσία (ν=5), ή/και υγρασιοθερμοθεραπεία (ν=5). Ποια θα είναι η Προσγείωση με τη Δρονεδαρόνη: Μια Αμιωδαρόνη χωρίς το Ιώδιο & τις Επιπλοκές του ή και χωρίς την Αποτελεσματικότητά της? Μελέτη ATHENA / Πόσο μακριά Είμαστε απο τον Εφιάλτη της ANDROMEDA? Το πρωτοπαθές καταληκτικό σημείο (πρώτη νοσηλεία για καρδιαγγειακό επεισόδιο ή θάνατος) ήταν ~32% για τη δρονεδαρόνη και ~39%για το placebo σε διάστημα 21 μηνών σε 2301 & 2327 ασθενείς με κολπική μαρμαρυγή αντίστοιχα (p<0.001). Βέβαια αυτή καθ’ευατή η θνητότητα δεν επηρεάστηκε (5% vs 6%). Η ομάδα της δρονεδαρόνης είχε υψηλότερη συχνότητα βραδυκαρδίας, παράτασης του QT, ναυτίας, διάρροιας, εξανθήματος και αυξημένων τιμών κρεατινίνης, αλλά όχι πνευμονικών και θυρεοειδικών παρενεργειών (μελέτη ATHENA/ NEJM 2009;360:668). Ωστόσο, είχε προηγηθεί η μελέτη ANDROMEDA (NEJM 2008;358:2678), η οποία είχε δείξει με το ίδιο φάρμακο αυξημένη θνητότητα σε ασθενείς με καρδιακή ανεπάρκεια και κλάσμα εξώθησης (ΚΕ) <35%, όπου βέβαια αντενδείκνυται πλέον η χορήγηση αυτού του πολυαναμενόμενου νέου φαρμάκου. Οι ίδιοι οι συγγραφείς της μελέτης ATHENA επισημαίνουν πως η περίοδος παρακολούθησης ήταν μόνον 21 μήνες και οι ασθενείς που παρουσιάζουν την πνευμονική κυρίως τοξικότητα στην αμιωδαρόνη την εμφανίζουν μετά τη διετία, και υποδεικνύουν επίσης πως το ποσοστό διακοπής του νέου φαρμάκου ανήλθε στο 30%. Επιτέλους κι ένα Φάρμακο στον Ορίζοντα να Αντικαταστήσει ίσως στο Άμεσο Μέλλον τα Κουμαρινικά Αντιπηκτικά στην Κολπική Μαρμαρυγή-Μελέτη RELY Η δαβιγατράνη σε μικρή (110 mg bid ) ή μεγάλη δόση (150 mg bid) φαίνεται πως έρχεται να αντικαταστήσει τα κουμαρινικά αντιπηκτικά και τον εφιάλτη με τo συχνό έλεγχο του INR σε ασθενείς με κολπική μαρμαρυγή. Στη μελέτη RELY σε 18113 ασθενείς με κολπική μαρμαρυγή έγινε τυχαιοποίηση μεταξύ ουαρφαρίνης και των δυο αυτών δόσεων δαβιγατράνης, με αποτέλεσμα η μικρή δόση να είναι ισοδύναμη με την ουαρφαρίνη, ενώ η μεγάλη δόση ίσως και ανώτερη της ουαρφαρίνης (NEJM 2009;361;1139). Η μικρότερη δόση ίσως να έχει και συνολικά λιγότερες αιμορραγικές επιπλοκές απο την ουαρφαρίνη (2.71% vs 3.36%). Ωστόσο, αναμένει κανείς περισσότερες πεπτικές διαταραχές με το νέο φάρμακο και δυστυχώς και περισσότερες αιμορραγίες απο το πεπτικό... (excerpt)

Ένας δομικός σχηματισμός σαν θύλακος που συχνά ανευρίσκεται στον αριστερό κόλπο (Εικόνα), είναι δυνητικά μια ισχυρή εστία θρόμβων που μπορεί να ευθύνεται για πολλά ισχαιμικά επεισόδια, ιδίως σε νέα άτομα, σύμφωνα με νέα δημοσίευση των Krishnan & Salazar από την California (JACC Cardiovasc Interv 2010;3:98-104). Στο θύλακο αυτό, όπως και στο ωτίο, μπορεί να λιμνάζει το αίμα και να δημιουργηθούν εκεί θρόμβοι με επακόλουθα θρομβοεμβολικά επεισόδια. Μάλιστα, αρκετά απο τα κρυπτογενή εγκεφαλικά επεισόδια μπορεί να οφείλονται σε αυτό το δομικό σχηματισμό. Οι συγγραφείς προτείνουν ότι ο αριστερός κολπικός θύλακος ανευρίσκεται περίπου στο 30% των ατόμων και πιστεύουν ότι αποτελεί ένα στάδιο στη φυσική πορεία της σύγκλεισης του ωοειδούς τρήματος. Ως γνωστόν στην πλειονότητα των ατόμων το ωοειδές τρήμα κλείνει, αλλά περίπου στο 25% παραμένει ανοικτό, θεωρούμενο ως παράγοντας κινδύνου για σοβαρές κλινικές καταστάσεις, όπως εμβολικά εγκεφαλικά επεισόδια, έμφραγμα μυοκαρδίου και επιπλοκές της πνευμονικής εμβολής. Το ανοικτό ωοειδές τρήμα (PFO) τείνει να έχει μειωμένη επίπτωση με την αύξηση της ηλικίας και απο ~34% στις πρώτες 3 δεκαετίες μειώνεται στο ~20% στις μεγάλες ηλικίες. Πιστεύεται ότι η φυσική σύγκλειση του PFO οφείλεται σε ανάπτυξη συμφύσεων, αλλά εάν η δημιουργία συμφύσεων περιοριστεί σε τμήμα μόνον του δομικού αυτού σχηματισμού, τότε η σύγκλειση είναι μερική και μπορεί έτσι να δημιουργηθεί ο αριστερός κολπικός θύλακος. Στην πλειονότητα των περιπτώσεων ο θύλακος ανευρίσκεται στον αριστερό κόλπο, και πιστεύεται ότι η διεργασία είναι συνεχής απο το PFO, στον αριστερό κολπικό θύλακο στην πλήρη φυσική σύγκλειση του PFO... Επέκταση των Επεμβάσεων Κατάλυσης της Κολπικής Μαρμαρυγής, αλλά με Μέτρια Αποτελεσματικότητα και Αρκετές Επιπλοκές

Μια νέα ανασκόπηση των επεμβάσεων κατάλυσης της κολπικής μαρμαρυγής (κΜ) σε διεθνές επίπεδο με τη συμπλήρωση ερωτηματολογίου δημοσιέυθηκε πρόσφατα απο τους Cappato & Συνεργάτες (Circ Arrhythm Electrophysiol 2010, 3:32). Στοιχεία συγκεντρώθηκαν από 85 απο τα 521 κέντρα που ερωτήθηκαν, δηλώνοντας τη διενέργεια 20825 επεμβάσεων σε 16309 ασθενείς με κΜ μεταξύ των ετών 2003 και 2006. Ο διάμεσος αριθμός επεμβάσεων αυτών των περιορισμένων σε αριθμό εξειδικευμένων κέντρων ανέρχεται σε 245 (διακύμανση, 2-2715). Όλα τα κέντρα περιέλαβαν παροξυσμική κΜ, 86% περιέλαβαν και εμμένουσα και 47% και χρόνια κΜ. Η καθοδόγηση των επεμβάσεων έγινε στην πλειονότητα με ηλεκτροανατομική χαρτογράφηση (Carto) σε 48% (περιμετρική κατάλυση) και με κυκλοτερή καθετήρα Lasso σε 27% των ασθενών (στομιακή απομόνωση των πνευμονικών φλεβών). Μετά απο 1.3 επεμβάσεις ανά ασθενή, η συνολική αποτελεσματικότητα ανήλθε στο 70% (διάμεση τιμή) χωρίς την ανάγκη χρήσης αντιαρρυθμικών φαρμάκων, ενώ άλλοι 10% των ασθενών παρέμειναν ασυμπτωματικοί μόνον με τη χρήση αντιαρρυθμικών φαρμάκων σε διάστημα 18 μηνών. Η επιτυχία ήταν σαφώς μεγαλύτερη σε ασθενείς με παροξυσμική κΜ (75% χωρίς και 83% με φάρμακα αντίστοιχα), σε σύγκριση με την εμμένουσα κΜ (65% & 75% αντίστοιχα) και τη χρόνια κΜ (63% & 72%). Μείζονες επιπλοκές παρατηρήθηκαν σε 741 ασθενείς (4.5%). Υπήρξαν 25 (0.15%) θάνατοι σχετιζόμενοι με την επέμβαση, 37 (0.23%) εγκεφαλικά επεισόδια, 115 (0.71%) παροδικά εγκεφαλικά, και 213 (1.31%) επεισόδια επιπωματισμού...((excerpt)

Είναι η Κολπική Μαρμαρυγή Φλεγμονώδης Νόσος?

Σε πρόσφατη δημοσίευση, έγινε διερεύνηση 3 δεικτών φλεγμονής, CRP, sICAM (soluble intercellular adhesion molecule), & ινωδογόνου, σε γυναικείο πληθυσμό (ν=24734) της μελέτης Women’s Health Study, που δεν είχε εξ αρχής κολπική μαρμαρυγή (κΜ) (Conen et al, Eur Heart J 2010, online:May 25, 2010). Σε διάρκεια 14.4 ετών, 747 (3%) γυναίκες εμφάνισαν το πρώτο επεισόδιο κΜ. Οι παραπάνω 3 δείκτες φλεγμονής συσχετίζονταν ατομικά και μετά απο διόρθωση για τους κλασικούς παράγοντες κινδύνου, με την εμφάνιση κΜ. Ο κίνδυνος κΜ ήταν ανάλογος του score φλεγμονής, βαθμιαία αυξανόμενος απο 1.66 σε 3.25 συμβάματα ανά 1000 άτομα-έτη απο το χαμηλότερο στο υψηλότερο score φλεγμονής (Εικόνα 1). Όπως τονίζουν οι συγγραφείς, ενώ έχει καταδειχθεί ότι η φλεγμονή μετά απο καρδιακή εγχείρηση μπορεί να οδηγήσει σε μετεγχειρητική κΜ, παραμένει υπο αμφισβήτηση εαν η φλεγμονή παίζει κάποιο ρόλο στην εμφάνιση της κΜ σε άλλες περιπτώσεις. Μαζί με αυτή τη μελέτη, 3 προοπτικές μελέτες κατέδειξαν ότι η CRP άμεσα συνδέεται με την επίπτωση κΜ (Cardiovascular Health Study & μελέτη Framingham), ωστόσο η ταυτόχρονη εξέταση πολλαπλών δεικτών φλεγμονής μπορεί να παράσχει καλύτερη εικόνα της φλεγμονώδους διασύνδεσης με την κΜ. Έτσι, διαφαίνεται ότι η χρόνια φλεγμονή μπορεί να έχει προ-αρρυθμική δράση και να οδηγήσει σε κΜ σε ευαίσθητα άτομα...(excerpt)

Σε πρόσφατη διπλή δημοσίευση (Circulation 2010;122 (suppl 2):S250 –S275./ Circulation 2010; 122 (suppl 3):S640 –S656) περιελήφθησαν οι νέες κατευθυντήριες οδηγίες ανάνηψης της Διεθνούς Επιτροπής Ανάνηψης (ILCOR) και της Αμερικανικής Καρδιολογικής Εταιρείας (AHA). Σε σύγκριση με τις προηγούμενες οδηγίες του 2005, σημαντικές πρόοδοι και εξελίξεις έχουν επέλθει και διαμείβονται στις νέες οδηγίες. Μια βραχύτατη περίληψη αυτών περιλαμβάνεται παρακάτω. ● Οι κοινοί διασώστες ξεκινούν την ανάνηψη εαν το θύμα δεν απαντά και δεν αναπνέει φυσιολογικά (αγνοώντας κάποιες αγωνιώδεις αναπνοές) χωρίς να ελέγξουν τον σφυγμό. ● Οι διασώστες ξεκινούν την ανάνηψη με καρδιακές μόνον μαλάξεις χωρίς τεχνητή αναπνοή ● Όλοι οι διασώστες (εκπαιδευμένοι και μη) πρέπει να παράσχουν ικανές καρδιακές μαλάξεις στα θύματα καρδιακής ανακοπής με ώσεις σε βάθος συμπίεσης τουλάχιστον 5 cm και συχνότητα τουλάχιστον 100/λεπτό, επιτρέποντας πλήρη επανέκπτυξη του θώρακα μετά απο κάθε μάλαξη και ελαχιστοποιώντας τις διακοπές των μαλάξεων ● Οι εκπαιδευμένοι διασώστες πρέπει να παράσχουν επίσης τεχνητή αναπνοή με αναλογία μαλάξεων-αερισμών 30:2 Επομένως αλλάζει η σειρά ανάνηψης απο ABC (airway-breathing-circulation/αεραγωγός-αναπνοή-κυκλοφορία) σε CAB (circulation-airway-breathing/ κυκλοφορία-αεραγωγός-αναπνοή (όπου κυκλοφορία βλ. μαλάξεις θώρακα). Το σύστημα επείγουσας βοήθειας θα πρέπει να ενεργοποιηθεί άμεσα σε κάθε περίπτωση που ανευρίσκεται άτομο που δεν απαντά. Dabigatran: An Alternative to Warfarin After Over Half a Century

For over half a century, warfarin or other vitamin K antagonists (VKA) (such as acenocoumarol, available in Greece and other countries), have been the gold standard and the only oral anticoagulants available which have been shown to effectively treat thromboembolism. However, their use has been plagued by inherent limitations with cumbersome monitoring via laboratory-guided adjustments of the dose, narrow therapeutic window, a lot of drug and food interactions and unpredictable and variable response. This has hindered patient compliance and has led to suboptimal therapy and poor anticoagulation control. Also patients unable or unwilling to take VKA have been offered no other choice of equivalent efficacy, i.e. until recently. Reasons for not receiving a VKA may comprise the following: drug allergy, patient refusal to take or decision to discontinue the drug, inability to maintain the international normalized ratio (INR) in the 2.0-3.0 range, physician decision as to the inappropriateness of receiving the drug, and/or inability to monitor the INR (lack of or difficulty in accessing a laboratory or lack of family support or assistance with this tedious task).These hurdles of conventional anticoagulant therapy have spawned efforts to develop new medications that will surpass these drawbacks while matching the efficacy of VKAs. Factor Xa has a strategic role in the coagulation cascade, critically poised at the juncture of the contact activation (intrinsic) and the tissue factor (extrinsic) coagulation pathways proximal to thrombin, both activating the final common pathway and leading to fibrin formation. Active factor Xa hydrolyzes and activates prothrombin to thrombin. Recently, cardiological societies in Europe (ESC), US and Canada (CCS) have updated their guidelines for atrial fibrillation (AF). However, there are several discrepancies which have been identified and discussed. Among them, the following are the most blatant ones. • Anticoagulation. Atrial fibrillation confers a 5-fold increase in the risk of stroke if left without anticoagulation therapy and a 2-fold increase in the risk of death from such thromboembolic strokes. The US guidelines recommend anticoagulation for a patient with a CHADS2 score of ≥2, and either aspirin or anticoagulation for a patient with a CHADS2 score of 1, while the CCS and ESC guidelines recommend anticoagulation for a patient with ≥1 risk factor(s). The new anticoagulants are taken into consideration by the CCS guidelines, recommending dabigatran instead of vitamin K antagonists (VKA). The ESC guidelines make recommendations for future use of dabigatran (when approved in the EU) depending on the risk of bleeding; for low-risk patients it may be considered, while for higher risk patients the lower dose of 110 mg of dabigatran may be considered. Also patients with a CHADS2 score of 1 may receive the lower dose (110 mg) of dabigatran. The US guidelines issued an update for dabigatran, indicating that it is a useful alternative to VKA... Dronedarone: the Hope and the Hype

Atrial fibrillation (AF) is the commonest cardiac arrhythmia afflicting ~1% of the total population in an age-dependent manner with 2.3% of individuals older than 40 years of age, or 5.9% of people older than 65. Approximately 70% of persons with AF are between 65 and 85 years of age. Patients with AF experience significantly higher mortality rates than patients without AF; adjusted relative mortality risk has been found approximately 20% higher in patients with AF in all age-sex strata during each of the 3 years studied (P < 0.05). One sixth of all strokes are attributable to AF and the percentage of strokes due to AF increases dramatically with age; of course the risk of stroke is significantly decreased with anticoagulation therapy by 60-70%, but this has its own inherent problems. Unfortunately, the use of antiarrhythmic drugs (AADs) employed thus far to maintain sinus rhythm is severely problematic with treatment being occasionally worse than the disease. Nevertheless, the goal is still to maintain sinus rhythm, since teleologically this is the optimal rhythm man is born and should live with, but the means to effect and sustain this, have inherent potentially prohibitive risks, as shown in the AFFIRM, RACE and other trials. However, even in the AFFIRM trial, which showed that AADs may be associated with increased mortality, overall maintenance of sinus rhythm (with or without AADs) was associated with improved survival compared with persistent AF. This observation supports the long-recognized mortality risk associated with AF, and hence the continued quest for development of new and safer antiarrhythmic agents and methods to maintain sinus rhythm. In this endeavor, newer pharmacological agents have recently emerged but not yet fulfilled this expectation. Left Atrial Appendage Closure: Feasible but Still Risky / Will it Though Obviate the Need for Anticoagulation?

Various studies and meta-analyses have suggested that in cases of left atrial thrombi in nonrheumatic atrial fibrillation (AF) patients, approximately 90% of them are located in the left atrial appendage (LAA). Patients are effectively protected from thromboembolism in this setting by anticoagulation therapy, shown to reduce the incidence by 60-70% with vitamin K antagonists and maybe more by the newer anticoagulants. However, anticoagulation therapy is limited by an increased risk of major bleeding, in addition to other hindrances, such as difficulty in monitoring anticoagulation therapy, several drug and food interactions for classical warfarin, or cost and bleeding issues with the newer drugs, all leading to limited use of anticoagulation in clinical practice with percentages reported at 30-50%. For these reasons, alternative device therapies with occlusion of the LAA, considered the most common source of thromboembolism in this cohort, have been recently pursued.In the European PLAATO (Percutaneous Left Atrial Appendage Transcatheter Occlusion) study, the first device ever used for this purpose, which, however, was subsequently withdrawn, had an initial implant success rate of 90% among 180 patients. However, there was a 1.1% mortality (2 patients) related to the procedure, while 6 cases of cardiac tamponade also occurred (3.3%). In two cases, surgical drainage of the tamponade was necessary (1.1%). In one patient the device embolized into the aorta after its release (0.6%) (at the end successfully snared and replaced). During follow-up, 3 strokes occurred (2.3% per year). The expected incidence of stroke according to the CHADS2-Score was 6.6% per year. Did PALLAS Deliver the Final Blow to Dronedarone?

The Permanent Atrial Fibrillation Outcome Study Using Dronedarone on Top of Standard Therapy (PALLAS) included patients aged over 65 years, who had at least 6-month history of permanent atrial fibrillation (AF) and risk factors for major vascular events. The latter comprised coronary artery disease, previous stroke or transient ischemic attack, symptomatic heart failure, which was defined as current New York Heart Association class II or III symptoms and admission to the hospital for heart failure in the previous year (but not in the most recent month), a left ventricular ejection fraction of <40%, peripheral arterial disease, or the combination of an age of 75 years or older, hypertension, and diabetes. They were randomized to receive either dronedarone at a dose of 400 mg twice daily or placebo. On July 5, 2011, the data monitoring committee recommended that the study be terminated for safety reasons. A total of 3236 patients had undergone randomization with median follow-up of 3.5 months. The investigators had defined as the first coprimary outcome: stroke, myocardial infarction, systemic embolism, or death from cardiovascular causes; and as a second coprimary outcome: unplanned hospitalization for a cardiovascular cause or death.The first coprimary outcome occurred in 43 patients receiving dronedarone and 19 receiving placebo (hazard ratio-HR, 2.29; P=0.002). There were 21 deaths from cardiovascular causes in the dronedarone group and 10 in the placebo group (HR, 2.11; P = 0.046), including death from arrhythmia in 13 patients and 4 patients, respectively (HR, 3.26; P = 0.03). Stroke occurred in 23 patients in the dronedarone group and 10 in the placebo group (HR, 2.32; P = 0.02). Hospitalization for heart failure occurred in 43 patients in the dronedarone group and 24 in the placebo group (HR, 1.81; P = 0.02). More Bleeding With the Newer Anticoagulants?

The new oral anticoagulants that are currently available are dabigatran (Pradaxa, Boehringer Ingelheim), a direct thrombin inhibitor, and the activated factor X (Xa) inhibitors, apixaban (Eliquis, Pfizer) and rivaroxaban (Xarelto, Bayer), with several other agents in the pipeline. These new oral anticoagulants are supposed to have a wider therapeutic window than warfarin, leading to a lower incidence of major bleeding. However, the results of large randomized trials indicate that bleeding remains a major concern even with the new agents. Studies with rivaroxaban, edoxaban and dabigatran showed that these drugs have incidences of severe bleeding comparable to those of enoxaparin and warfarin. The number of bleeding events is rising due to the ageing of the population and the increasing need for interventional therapies. The shorter half-life of the new agents might facilitate the management of bleeding events and the control of anticoagulation during interventions or emergency circumstances. If bleeding occurs, the lack of specific antidotes limits the therapeutic options. Due to the selection bias in the initial randomized studies, the absolute bleeding risk might be underestimated. This can only be measured after exposure of the drugs to larger populations with close post marketing surveillance within registries, a strategy that would be very helpful in defining the actual bleeding risk for the new drug classes. In case of bleeding occurrence, supportive care should be sufficient for most patients because of the short duration of action of the new agents. Biological Pacemaker: Science Fiction or a Feasible Project?

Artificial electronic pacemakers have been successfully driving the hearts and reliably saving the lives of millions by providing cardiac pacing for a variety of cardiac bradyarrhythmias. However, there are several caveats to this technology not merely limited to procedural and hardware complications associated with its usage. These hurdles have prompted research in the development of a biological counterpart which could replace or supplement its electronic version. The concept of the biological pacemaker is very appealing, albeit most challenging. If normal working myocytes or conduction system cells could be transformed to perform the pacemaker function, there would be no need to replace pulse generator, and no issues with circuit or lead failure, size mismatch or foreign body infection would ever arise. Two general approaches have been pursued in the development of biological pacemakers, gene transfer into cardiac myocytes to create or enhance the pacemaker function, and cell transplantation into the heart that can perform the pacemaker function on their own or in conjunction with native cardiac myocytes. There are always pros and cons of these different approaches. Importantly, in order to develop such a biologic pacemaker, two elements are deemed essential: to select a gene that can direct cells of non-automatic tissue to induce spontaneous phase 4 depolarization in a reliable and automatic manner, similar to the native sinus node cells or other conduction tissue with inherent automaticity; and subsequently to develop a technique to deliver this gene into the target tissue. Gene transfer methods may use injection of adenoviral vectors, of genetically engineered stem cells or use of gene-carrying mesenchymal stem cells. Thus, biologic pacing could be accomplished either by genetic engineering, cellular therapy or a combination of both. Over the past decade, gene therapy has been explored to upregulate β2-adrenergic receptors, to downregulate inward rectifier current, and to overexpress pacemaker current as potential sources of biological pacemakers. Focal Rotor/Driver Ablation for Atrial Fibrillation: a Dream Come True?

The success of catheter ablation of atrial fibrillation (AF) remains modest despite the use of a variety of techniques, because the real target remains elusive. Even the endpoint of AF ablation is still under debate. It is not yet known whether isolation of the pulmonary veins (PVI), currently having a reproducible modest success rate when performed at the antrum and guided by electrophysiological mapping, is what is needed or additional foci should be routinely targeted. Some investigators have suggested that it is the ganglionated plexi (GPs) that mostly provide the triggers to the pulmonary veins, and GPs should be the primary targets during ablation. Others have focused on ablation of complex-fractionated atrial electrograms (CFAEs) claiming successful outcome; some have combined PVI with ablation of CFAEs. In cases of persistent AF, additional ablation lines and/or posterior atrial wall debulking have been applied to enhance the success rate of the procedure. Until recently, no focal ablation was ever reported to have any success in this patient population.Recently, Narayan et al devised an optical computational approach and mapped localized electrical rotors and focal impulse sources in 49 patients with AF. Patients with persistent AF (n=30) had more sources than those with paroxysmal AF (2.1±1.0 vs 1.5±0.8, P=0.02), related to shorter cycle length (163±19 ms vs 187±25 ms, P<0.001). The majority of sources were found in the left atrium and only about a quarter of sources were localized in the right atrium. In a subsequent case report, the authors presented a patient with persistent AF who failed prior left atrial radiofrequency Maze and endocardial ablation procedures. Use of this novel computational mapping disclosed one AF rotor in the right atrium and another in the left atrium. Brief 3–5-minute ablation applications at each rotor terminated AF, which remained noninducible on testing, while no AF recurrence was reported over the next 6 months. In another study by the same group, the investigators localized rotors or focal sources (mean 2.1 sources) in 98 (97%) of 101 patients with sustained AF and then proceeded with ablation aiming at these targets. Leadless Pacing: The Future is not Here Yet!

Despite great technical advances over the last decades, cardiac pacing is still associated with a significant rate of complications mostly ascribed to the pacing leads. Leads are susceptible to mechanical stress, whether transvenous or epicardial, and constitute the pacing component most prone to failure; they are the major contributor to compli-cations, may cause vascular obstruction, and other late complications, such as mechanical failure and infection, whereby extraction is required with its attendant dire consequences. Implantation of a pacemaker in young patients exposes them to high risk of subsequent lead complications, and when failed leads are replaced without extraction, the presence of multiple endocardial leads may cause major vascular morbidity. New devices for bi-ventricular or bifocal pacing to effect cardiac resynchro-nization therapy (CRT) incorporating 3 leads are associa-ted with even more problems. Hence, there came about a resurge of interest in the technology of leadless pacing.The first totally self-contained leadless pacemaker system was proposed by Spickler back n 1970 using a device powered by mercury-zinc and nuclear power that was successfully tested in animals. Almost 30 years later, Gotto et al tested an automatic power-generating system (AGS) which converts kinetic into electric energy for quartz watches as a power source for cardiac pacemakers. They could demonstrate that the circuit generated pulses of 0.5 ms width at 1 Hz (60 pulses/min). The voltage of the AGS was maintained at 1.6 V while it was being charged by the accelerations. The generator supplied pulses of 0.75 V, 1.47 mA via a 510-ohm load. With fully charged AGS, the generator was also used to pace a mongrel dog's heart at 140 beats/min for 60 min. During pacing, the AGS supplied 420 mJ to the circuit and the cardiac muscle. The AGS was placed on the right ventricular wall of the dog under anesthesia. Energy of 80 mJ is stored in a capacitor by the heart beating at ~ 200 beats/min for 30 min. Thus, the AGS generated 13 microJ per heart beat. Vitamin D exists in several forms comprising steroid-like fat-soluble molecules (secosteroids). Cholecalciferol (vitamin D3) is synthesized in the skin in response to ultraviolet irradiation producing photochemical cleavage of a cholesterol precursor (7-dehydro-cholesterol). Irradiation of ergosterol, a membrane sterol found in the Ergot fungus, produces ergocalciferol (vitamin D2). Dietary sources of vitamin D include fish oils (D3), egg yolk (D3), mushrooms (D2), and fortified cereals and dairy products (D2 or D3). Vitamin D derived from all different sources undergoes two successive hydroxylation steps, first in the liver (25-hydroxyvitamin D) and then the kidney (1, 25-hydroxyvitamin D2 / calcitriol). The former form has a longer half-life than the latter (weeks vs. several hours). The biologic effects of vitamin D result largely from its binding to the vitamin D receptor (VDR), a nuclear steroid hormone found in almost every tissue. Calcitriol (D2) appears to have the greatest affinity for the receptor. The prevalence of vitamin D deficiency is estimated around 30–50% in the adult population in developed countries, mostly the result of insufficient cutaneous production due to decreased exposure to sunlight, and to a less degree from low dietary intake. Serum levels of 25-hydroxyvitamin D >30 ng/mL are considered adequate, while levels < 20 ng/mL are diagnostic of vitamin D deficiency.The endocrine functions of vitamin D in relation to bone metabolism and mineral ion homoeostasis are well known. Vitamin D deficiency results in reduced intestinal absorption of calcium, which stimulates the production of parathyroid hormone with an ensuing accelerated bone de-mineralization to maintain serum calcium concentration, with all these alterations leading to the clinical effects of hypocalcemia. Obstructive sleep apnea (OSA) has been linked to hypertension in several experimental, epidemiological, and clinical studies. Animal modes of sleep apnea have provided strong evidence for a causal relationship with hypertension. Studies in humans have demonstrated that patients with sleep apnea have an increased blood pressure and a higher incidence of hypertension. The most compelling evidence linking OSA and hypertension was provided by data from the Wisconsin Sleep Cohort Study. This study has demonstrated a dose-response association between sleep-disordered breathing at baseline and the presence of de novo hypertension 4 years later. The odds ratios for the presence of hypertension at the 4-year follow-up study according to the apnea-hypopnea index (AHI) at base line were estimated after adjustment for baseline hypertension status, body mass index (BMI), neck and waist circumference, age, gender, and weekly use of alcohol and tobacco. Relative to the reference category of an AHI of 0 events per hour at baseline, the odds ratios for the presence of hypertension at follow-up were 1.42 (95% confidence intervals-CI 1.13-1.78) with an AHI of 0.1-4.9 events per hour at base line as compared with none, 2.03 (95% CI 1.29-3.17) with an AHI of 5.0-14.9 events per hour, and 2.89 (95% CI 1.46-5.64) with an AHI of 15.0 or more events per hour. These findings suggest two important concepts. First, sleep-disordered breathing is a risk factor for hypertension in the general population. The cardiovascular system undergoes significant changes during pregnancy to adapt to and accommodate the increased metabolic demands of the fetus and the mother. These adaptations produce an important hemodynamic burden on patients with underlying heart disease, and confer an increase in morbidity and mortality. Furthermore, pregnancy may cause specific cardiovascular disorders, which can impose a risk to the pregnant woman and to her fetus. It is estimated that in the western world 0.2-4% of all pregnancies are complicated by cardiovascular diseases (CVD). This risk is in the ascending order as the age of first pregnancy is increasing and as the number of cardiovascular risk factors is rising (e.g. smoking, hypercholesterolemia, diabetes, hypertension, obesity). During pregnancy, the most frequent cardiovascular events relate to hypertension (6–8%). On the other hand, in the western world, the most frequent CVD present during pregnancy is congenital heart disease-CHD (circa 75%), while rheumatic heart disease predominates in the other countries (circa 70%) and CHD is seen in ~15%. In pregnant women with heart disease, maternal death is estimated around 1% but it varies depending on the underlying CVD; neonatal complications occur in 20–28% and neonatal mortality ranges between 1% and 4%. In general, CVDs are the most common cause of maternal death during pregnancy in the Western industrialized world.4 Thus, women of child-bearing age with CVD or cardiovascular risk factors should be counseled and managed early by an interdisciplinary team of gynecologists, cardiologists, and, when necessary, cardiothoracic surgeons... Totally Implantable Artificial Heart: Still a Major Challenge

The first mechanical heart was placed by Liotta and Cooley in 1969 in a dying patient at the Texas Heart Institute in Houston as a 2 ½-day bridge for a transplant, albeit the patient died 32 hours after transplantation.1 Years later (1982) a totally implantable artificial heart (model Jarvik-7) was permanently implanted in a patient by DeVries et al at the University of Utah Medical Center in Salt Lake City, Utah, USA and the patient lived for 112 days.2 Subsequent attempts of implantation of a total artificial heart (e.g. CardioWest/SynCardia models) have limited its use as a bridge to transplantation, like the left- or bi-ventricular assist devices (VADs).3-7 The SynCardia model (SynCardia Systems Inc., Tuscon, AZ) has been approved for compassionate use by the Food and Drug Administration (FDA) for patients with end-stage biventricular heart failure as a bridge to transplantation since 1985 and has had FDA approval since 2004.3-5 The SynCardia™ total artificial heart, weighing 180 g, providing a stroke volume of 70 cc, is a pneumatically driven, pulsatile system capable of flows of >9L/min. It is indicated for temporary use as a bridge to transplantation in patients with end-stage non-reversible bi-ventricular failure. Currently, the recipients of this device are hospital-bound and attached to a large pneumatic driver. The bridge to transplantation rate has been ~80% in >1100 implants. Renal Denervation for Resistant Hypertension: Blinded or Unblinding Recent Trial?

Renal artery sympathetic denervation, performed via a percutaneous catheter ablation procedure applying radiofrequency energy, has been developed to address renal sympathetic overactivity as a pivotal mechanism in the pathophysiology of hypertension.1 In 2009, the first-in-man experience with this procedure was published.2 Several studies followed, including the Symplicity HTN-1 trial (n=45), the randomized Symplicity HTN-2 trial (n=106) and the expanded open-label Symplicity-2 study (n=153).3-6 However, all the initial evidence suffered from important limitations related to small cohort size, short follow-up and by and large incomplete data.1 According with a review and meta-analysis of 12 studies, most of them being observational studies without a control group, renal denervation in a total population of 561 patients with resistant hypertension, resulted in a significant reduction in mean blood pressure at 6 months.7 In the controlled studies (2 randomized controlled trials, n=133; and 1 observational study with a control group, n=50), there was a reduction in mean systolic and diastolic blood pressure at 6 months of –28.9 mm Hg and –11.0 mm Hg, respectively, compared with the control groups (p < 0.0001). In the uncontrolled studies (n=396), there was a reduction in mean systolic and diastolic blood pressure at 6 months of –25.0 mm Hg and –10.0 mm Hg, respectively, compared with the pre-procedural values (p < 0.00001). Statins are molecules of fungal origin, which inhibit the hydroxymethylglutaryl-CoA (HMG-CoA) reductase enzyme, a key step in the sterol biosysnthesis, rendering them powerful cholesterol lowering medications contributing to significant prevention of cardiovascular disease.1 Statins are characterized by differences in bioavailability, lipo/hydrophilicity, cytochrome P-450 mediated metabolism and cellular transport mechanisms, differences that are reflected in their relative capacity in LDL-cholesterol lowering and possibly in parenchymal or muscular toxicities.2 Statin intolerance comprises, among others, musculoskeletal problems, such as statin-induced muscle and tendon disorders which are the most common cause of statin discontinuation.3-10Musculoskeletal ProblemsStatin therapy has long been associated with musculoskeletal (MS) problems in approximately 10% - 25% of patients treated in real-world clinical practice, but such problems have rarely been reported in controlled clinical trials,3,4 and their incidence has thus far been underestimated.5 Studies have concentrated on creatine kinase (CK) elevations to identify myopathy. However, many patients can have normal serum CK levels despite myalgia and persistent weakness and muscle biopsy - proven myopathy. Discontinuation of statin and rechalllenge may be required to prove that it is statin-related. The American Heart Association (AHA) and the American College of Cardiology (ACC) have just issued (September 23, 2014) new guidelines for the management of patients with non-ST elevation (NSTE) acute coronary syndromes (ACS).1 They start with definitions. Absence of persistent ST elevation defines NSTE-ACS (except in patients with true posterior myocardial infarction –MI). Further classification of NSTE-ACS is based on whether cardiac troponin is elevated (NSTEMI) or not (unstable angina - UA). ST depression, transient ST elevation, and/or T-wave inversion may be present but are not required for a diagnosis of NSTEMI. Abnormalities on the electrocardiogram (ECG) and elevated cardiac troponins in isolation are insufficient to make the diagnosis of ACS but must be interpreted in the appropriate clinical context.1,2 Thus, UA and NSTEMI differ primarily by whether ischemia causes myocardial damage with a detectable cardiac biomarker. A variety of dietary supplements have been proposed for the management of hypertension and other cardiovascular diseases, but the evidence for their efficacy is meagre.1 However, over the last few years, there is growing interest in the potential benefit of tea in cardiovascular protection. 2-4 Tea is the extract of Camellia sinensis, and one of the most widely enjoyed drinks.2,3 The leaves of the plant are rich in flavonoids, containing several phytochemicals including phenols and catechins, to which potent antioxidant, free radical scavenging, appetite-modifying and hypolipidemic effects have been ascribed.2-7 Long-term consumption of beverages containing catechins inhibits the formation of oxidized lipids and decreases body fat.5-9 Over 70% of flavonoids in green tea are catechins. Epigallocatechin-3-gallate (EGCG) is the cardinal (65%) catechin in green tea. In-vitro studies have shown that the epigallocatechins in green tea have angiotensin converting enzyme inhibitor properties,10 and vasodilatory actions. 11 Molecular and animal studies have demonstrated that green tea catechins enhance processes, thought to stimulate bile acid production, decrease cholesterol concentration in the hepatocytes, inhibit intestinal absorption of lipids and upregulate low-density lipoprotein (LDL) receptors in the liver, mechanisms all leading to favorable blood lipid profile. 2-4,12,13Black tea also contains small amounts of catechins. A plethora of studies have provided convincing evidence for a high risk of adverse cardiovascular events associated with NSAIDs related to a number of risk factors, which are herein briefly reviewed and an algorithm is suggested for their safer use.

Whether anticoagulation, rhythm control strategies and other interventions aiming at preventing thromboembolic events and ameliorating the clinical outcome of AF patients, may also have a beneficial effect on long-term cognitive function remains to be seen in future studies.

The End of the Digoxin Era?

Digoxin is one of the oldest of cardiovascular drugs which is still frequently used, both in patients with atrial fibrillation (AF) and patients with heart failure with or without AF. The use of digoxin preceded the era of evidence based medicine. However, over the recent past, there has been growing evidence disputing and challenging the safety and efficacy of digoxin, while evidence has accumulated that a plethora of other therapies for both heart failure and atrial tachyarrhythmias has proven more effective and safe. Nevertheless, digoxin still retains its role, albeit limited, in the current era, but most recent evidence has cast significant doubts about its safety. Thus, its role remains controversial and the drug should be reserved for specific patients and clinical scenarios, with careful monitoring of its serum concentration due to its narrow therapeutic and toxic ranges, maintaining it <0.8 ng/mL, with additional monitoring of serum electrolytes and renal function to avoid potential confounders that may enhance the proarrhythmic risk and susceptibility to digoxin toxicity.

Over the recent years, new oral anticoagulant agents have been developed and entered the clinical arena, the direct or non-vitamin K oral anticoagulants (NOACs). Although more expensive, these agents have been proven as equivalent or superior to vitamin K antagonists in the treatment of non-valvular atrial fibrillation (AF) and venous thrombo-embolism with a lower incidence of intracerebral hemorrhage. Nevertheless, occurrence of major or life-threatening bleeding events is still quite possible. Thus, there is a major concern regarding the lack of a specific antidote and rapid reversal agent in such disastrous situations. Fortunately, the development of effective specific NOAC antidotes has recently made great advances, which are herein briefly reviewed.

The new Dietary Guidelines for Americans 2015-2020 focus on healthy eating patterns and exercise. They recommend 3 different healthy eating patterns: the Healthy US Style, the Healthy Mediterranean Style, and the Healthy Vegetarian Style eating pattern, accommodating a variety of cultural and individual preferences, but sharing many common food based features. A healthy eating pattern includes a variety of vegetables, fruits, grains, low-fat dairy products, protein foods, and oils. Specific recommendations have been made for particular dietary components to reduce: saturated fat (< 10% of calories); sodium (< 2.3 g/day); and added sugars (< 10% of calories). Interestingly, these guidelines do not make a recommendation for a quantitative limit for dietary cholesterol indicating the lack of adequate evidence for such recommendation. However, they state that there is evidence showing that eating patterns that include lower intake of dietary cholesterol (~100 - 300 mg/d) are associated with reduced risk of cardiovascular disease, and risk of obesity. Finally the new guidelines emphasize the importance of physical activity and exercise to promote health.

Oral anticoagulants (OAC) decrease the thromboembolic risk of non-valvular atrial fibrillation (AF) at the expense of increased bleeding. Over the years, several risk stratification schemes for both stroke and bleeding risk have been devised, among which lately the respective CHA2DS2-VASc and HAS-BLED scores predominate. However, even when the bleeding risk score is high, the guidelines recommend not to withhold OAC at least for patients with high stroke risk, but to attempt to concomitantly modify the conditions contributing to the high bleeding risk. The CHA2DS2-VASc score has been considered more reliable than other scores in identifying “truly low-risk” patients who do not require OAC, in whom the risk of bleeding may negate the protective effect of OAC. Some have suggested more complex schemes to better identify very low risk patients, but these schemes may lead to more extensive and costly assessments to decide on a relatively simple question, i.e. the need or not for anticoagulation therapy. In the era of non-vitamin K oral anticoagulants (NOACs), this may not be necessary any more, and a simple recommendation of providing every AF patient with OAC therapy may turn out to be a more practical and realistic approach, as long as these newer agents remain safe and effective. A permanent pacemaker is commonly required in patients undergoing transcatheter aortic valve implantation (TAVI) at ranges up to 30-50%. In general, the incidence is higher with the self- vs balloon-expandable valves. Several risk factors have been identified. Importantly, pacemaker implantation does not seem to improve prognosis and this needs to be further explored. Risk stratification for sudden cardiac death in non-ischemic dilated cardiomyopathy remains a difficult and controversial issue. This is currently guided by left ventricular ejection fraction, severity of heart failure symptoms according to New York Heart Association classification, and the morphology and duration of the QRS complex. The results of a recent study stirred some initial controversy with regards to the utility of the implantable cardioverter defibrillator in these patients, however, a subsequent meta-analysis confirmed prior findings of the survival-prolonging benefit of device therapy. Contemporary technology using mapping tools and algorithms in animal and computer models has suggested discrete atrial fibrillation (AF)-sustaining mechanisms in the form of electric rotors as focal areas of AF perpetuation. When these focal sites were targeted, ablation initially appeared promising in reducing the recurrence rates of AF. However, many other investigators have been unable to reproduce this kind of success with focal impulse and rotor modulation (FIRM)-guided ablation, and have cast doubts on the effectiveness of this novel technique that could have led to a paradigm-shift in our approach to AF ablation. A single randomized trial that was recently published in a high-impact cardiology journal indicating poor results of rotor ablation was subsequently retracted amidst a cloud of controversy and doubt about the flaws and conflicts of current medical publishing. Thus, until further evidence from ongoing and future randomized studies becomes available, the target of focal ablation for AF patients continues to remain elusive. Rhythmos 2017;12(2):21-25.

Bicuspid aortic valve (BAV) disease is the most common congenital cardiac malformation with a prevalence of 0.4% to 2%. For a long time, a BAV has not been considered an indication for transcatheter aortic valve implantation (TAVI) due to insufficient technology and poor procedural results conferred by a challenging valvular anatomy leading to poor stability of the prosthetic valve and/or paravalvular regurgitation due to distorted native valve leaflets. Large randomized controlled TAVI trials typically excluded bicuspid aortic stenosis (AS) because of its unique anatomic features. However, current technological advancements are apparently changing this landscape, and over the last few years, TAVI appears technically feasible, albeit a demanding procedure, and has been used to treat severe bicuspid AoV stenosis with promising results, as shown, apart from earlier case reports and patient series, in several recent observational studies and comparative trials between patients with tricuspid and patients with BAV. Thus, current literature cautiously supports a role of TAVI in selected BAV AS patients; however, the final role of this technique in this challenging group of patients will need to be determined from randomized controlled trials comparing TAVI with surgical replacement and studies comparing TAVI in bicuspid and tricuspid aortic valves. According with the new European and American guidelines, the role of an electrophysiology study (EPS) remains small for risk stratification for sudden cardiac death (SCD), limited to patients with an LVEF >35%, mostly indicated when evaluating symptoms suggestive of ventricular tachyarrhythmias, including palpitations, presyncope and syncope, mainly in those with underlying structural heart disease, particularly coronary artery disease in the setting of a remote myocardial infarction rather than non-ischemic cardiomyopathy. Importantly, there is no indication or need of EPS as a prerequisite for any of the standard indications of implantable cardioverter defibrillator (ICD) for secondary or primary prevention of SCD. The first generation bioresorbable scaffolds (BRS) did not deliver their promise for reduced risk of late stent thrombosis and neoatherosclerosis forming inside the stent and restoration of endothelial vasomotion. To our chagrin, the incidence of early, late and very late scaffold thrombosis was higher than conventional metallic stents leading to increased rates of adverse cardiovascular events and outcomes. Unfortunately, it took a decade to fully appreciate these major drawbacks. The manufacturer of the first, apparently hastily approved BRS in both Europe and the USA discontinued its production in September 2017. Nevertheless, hope remains and newer generation BRS are already in the pipeline expecting that improved technology and implantation strategies may overcome these severe limitations and finally recredit and reinstate the BRS concept. The results of the CABANA trial were recently presented at the Heart Rhythm Society Meeting in May 2018, indicating that ablation in patients with atrial fibrillation (AF) did not confer a benefit over drug treatment in the intention-to treat analysis, but did so in an on-treatment analysis. The presentation stirred commotion in the medical community with fierce controversy appearing in the media, mostly related to an apparent willingness of electrophysiologists to dispel the first and accept the second type of analysis. Sugary Beverages Pose Significant Risks to Cardiovascular and Overall Health

Consumption of added sugar is associated with weight gain and metabolic syndrome, but most importantly with cardiovascular disease (CVD) morbidity and mortality. Sugar-sweetened beverages (SSBs) in the form of soft drinks or sodas constitute a major source of added sugar with its attendant ill effects on health. Of particular concern is the rising consumption of soft drinks among young people. The World Health Organization (WHO) recommends the reduction of free sugars to <10% of total energy intake due to their potential implications in dental caries, weight gain and CVD consequences, however, adherence to this recommendation is generally very low. Non-caloric drinks in the form of artificially sweetened beverages (ASBs) (diet sodas) are popular as alternatives to SSBs. However, these sweeteners may also have various negative, albeit at a lesser degree, health outcomes, including weight gain, obesity, metabolic syndrome, type II diabetes, and CV events. Finally, substantially lower health risks are incurred from 100% fruit juice consumption compared with SSBs. All these issues are herein reviewed and measures are discussed to lessen the consequences of such an unhealthy dietary behavior. Rhythmos 2019;14(3):45-50.

Use of Sodium–Glucose Cotransporter 2 (SGLT-2) Inhibitors Beyond Diabetes: On the Verge of a Paradigm Shift?

The sodium–glucose co-transporter 2 (SGLT2) inhibitors have proven effective in glycemia control in patients with type 2 diabetes (T2D) by increasing urinary glucose excretion. However, the beneficial effects of SGLT2 inhibition extend beyond glycemic control, with new studies demonstrating beneficial effects that lead to improved cardiovascular (CV) (cardioprotection) and renal outcomes (renoprotection) in patients with T2D. Pivotal CV outcomes trials have demonstrated a 27-35% reduction in heart failure (HF) hospitalizations in patients with T2D. Importantly, a variety of pleiotropic effects of these new agents have been identified that include, but are not limited to, anti-atherosclerotic, anti-inflammatory, and anti-oxidant effects, decreased vascular stiffness and improved endothelial function, weight loss, reduction in sympathetic activity and in cardiac arrhythmogenesis. Ongoing studies are investigating these actions in patients with and without diabetes. Such results, if positive, may lead to a paradigm shift in the management of CV, renal and even other diseases beyond diabetes. Rhythmos 2020;15(1):67-71.

During the COVID-19 pandemic, a Kawasaki-like multisystem inflammatory syndrome with predominant myocarditis associated with SARS-CoV-2 infection has recently been reported, now referred to as Multisystem Inflammatory Syndrome in Children (MIS-C) Associated with COVID-19, or Pediatric Inflammatory Multisystem Syndrome Temporally associated with SARS-CoV-2 infection (PIMS-TS). These pediatric patients present with features that overlap features of Kawasaki Disease (KD) and toxic shock syndrome, albeit with distinct differences that relate to more common myocardial involvement requiring vasopressor support in PIMS-TS/MIS-C compared to KD (~50% vs ~5%), and less common coronary artery aneurysms (~9% vs ~25%). PIMS-TS/MIS-C seems to be largely immune-mediated, triggered by COVID-19, with the induced hyperinflammatory syndrome possibly due to post-infectious cytokine storm, rather than a result of direct cell injury caused by the virus. Treatment of this syndrome is similar to KD and includes intravenous immunoglobulin (IVIG), corticosteroids, and antiplatelet agents, while a few patients have also received interleukin-6 (IL-6) inhibitors. A sizeable proportion of individuals contracting the corona virus disease 2019 (COVID-19) infection remains asymptomatic, while some patients develop mild symptoms and others exhibit severe symptomatology becoming critically ill necessitating admission to intensive care unit exposed to a high mortality risk. The pathophysiological mechanisms underlying this diversity in the clinical picture of COVID-19 are poorly understood. A variety of reasons have been postulated, among which, viral load, age, gender, immune response, blood type, genotypes, polymorphisms, comorbidities and pre-existing immunity are actively explored. Thus, there is a dire need to further elucidate this phenomenon and find ways to identify and discern susceptible from resistant individuals with the ultimate goal to find a cure for the disease. COVID-19 Vaccines: Cardiovascular Perspectives: COVID-19 Vaccines

The accelerated development of COVID-19 vaccines has ushered in new hope for the beginning of the end of this unprecedented pandemic with the heaviest toll on global health and economy. The goal of vaccination is to achieve “herd immunity” which helps interrupt the chain of transmission with the threshold of such immunity estimated at 60-70%. There are four main types of COVID-19 vaccines: messenger RNA (mRNA); viral vector; protein-based; and whole virus. mRNA vaccines and most of the COVID-19 vaccines are using various versions of the spike (S) protein as their vaccine antigen component, while a few vaccines employ the whole virion. Local reactions and mild symptoms are the most common adverse reactions to vaccination; however, anaphylaxis is a potentially life-threatening adverse effect and needs to be monitored and promptly managed. Deaths with temporal association with vaccination have also been reported, but not causally linked to vaccination. Thrombotic events have also been reported, particularly with two brands, and have caused alarm but apparently remain extremely rare, nevertheless authorities remain watchful. One is considered fully vaccinated for COVID-19 ≥2 weeks after one has received the last dose. Unfortunately, new strains of COVID-19 are emerging fast, for which the current vaccines may be less effective. Thus, it is still crucial to continue wearing facemasks, apply hand washing, and social distancing in order to slow viral spread and to protect everybody from infection. Several unknowns still remain about COVID-19 vaccines that relate to the safety and efficacy of the vaccines in “special” populations, the degree and duration that these vaccines protect against infection and transmission. and possible long-term adverse effects of vaccination, not yet encountered in phase 3 trials. Interindividual clinical variability characterizes COVID-19 infection, ranging from no or mild symptoms in >95% of individuals to severe and life-threatening acute respiratory distress syndrome with bilateral pneumonia requiring intensive care unit monitoring in <0.5% of infected individuals. The host genetic background seems to determine the susceptibility and outcome in COVID-19 patients. Several relevant genetic variants and risk genes have been identified, relating to blood type, HLA system, angiotensin converting enzyme and other proteins, cytokines, and other host genetic signals and immune system’s specific response. On the other hand, emerging new variants of the COVID-19 virus with enhanced transmissibility/infectivity and immune escape ability pose new risks, especially if they are going to have an impact on the efficacy of currently available vaccines. Currently available COVID-19 vaccines may confer several side-effects, including cardiovascular (CV) adverse effects. Among them, perimyocarditis, acute myocardial infarction, arrhythmias and vaccine-induced immune thrombotic thrombocytopenia (VITT) with multiple vascular beds involved causing cerebral venous sinus thrombosis, splanchnic vein thrombosis, deep vein thrombosis and pulmonary embolism, as well as arterial thrombosis, constitute the most worrisome CV complications. Vaccine breakthrough COVID-19 infections are increasingly occurring in fully vaccinated individuals, who may also spread COVID-19 to others. Hence, one is better protected if one wears a mask and maintaining distancing when in indoor public places. Mild infections pose little risk to vaccinated individuals and their contacts, while they may boost the individual immune responses and thus necessitate only monitoring as a precautionary measure. However, higher infectiousness and transmissibility of viral variants remain worrisome. Variants of concern are overrepresented among post-vaccination breakthrough COVID-19 infections. Identifying subgroups at a high risk for severe breakthrough infections is important in prioritizing early preventive treatment or prophylaxis. Despite a strong protection afforded by vaccination against severe disease, breakthrough infections may still advance to severe or critical illness at not-insignificant rates. The age distribution of patients with severe breakthrough infections is skewed towards older age groups and individuals with underlying comorbidities. All these issues and factors modulating the probability of a breakthrough COVID-19 infection in vaccinated people are herein discussed, and a tabulated list of the recommendations of the World Health Organization on COVID-19 infection prevention and control is also presented. A recent surge of COVID-19 reinfections has been ascribed to new variants of concern (VOCs) with increased transmissibility, such as the Omicron variant, but also to laxity in compliance with measures of prophylaxis, incomplete or lack of vaccination, the Peltzman effect where people tend to increase their risky behavior when safety measures are mandated, weaker antibody responses after booster immunization in some individuals and/or immunocompromised patients, and increased antigen variability in VOCs. The latter impairs humoral and cellular immunity post-infection rendering COVID-19 convalescents more vulnerable and at risk of re-infection with VOCs. Unfortunately, COVID-19 disease is here to stay demanding observance of measures of prophylaxis, expansion of vaccination programs and continued vigilance; there is still a dire need for the development of durably effective vaccines against SARS-CoV-2 but also against its emerging variants. Cardiac resynchronization therapy (CRT) via biventricular pacing (BiVP) has benefitted a large group of heart failure (HF) patients with low (<35%) left ventricular (LV) ejection fraction (LVEF) and cardiac dyssynchrony, mostly in the form of left bundle branch (LBB) block, conferring amelioration of their HF symptoms and prolongation of their survival. However, it entails a tedious procedure to place and find a stable and functional position of the LV lead in a coronary sinus tributary, which may fail in a considerable percentage of patients, while up to one third of patients, even then, may turn out to be non-responders. Over the recent years, strong new data from observational studies and meta-analyses have shown the safety and feasibility of LBB area pacing (LBBAP) in patients with bradyarrhythmias and most importantly in HF patients in need for CRT. LBBAP yields satisfactory pacing threshold and R wave sensing and low complication rates. Particularly, in patients with CRT indication, LBBAP, as an alternative approach to CRT, has shown significant improvement of functional class and LVEF during short-and mid-term follow-up. Thus, LBBAP, as a relatively novel CRT modality, demonstrates a most promising potential (equivalent or even superior) role for effective CRT for HF patients in need of a viable alternative to BiVP, and also circumvents certain limitations of His bundle pacing, which is another, albeit potentially problematic, mode of CRT. Nevertheless, we are still in dire need for confirmatory data from properly designed randomized controlled trials to further establish the role and value of this alternative CRT pacing modality.
