Ο Ρόλος της Φλεγμονής στην Κολπική Μαρμαρυγή

Η κολπική μαρμαρυγή (κΜ) είναι η πιο συχνή εμμένουσα αρρυθμία στην κλινική πράξη, καθώς παρουσιάζεται περίπου στο 1% των ατόμων ηλικίας ≤65 ετών και στο 5% των ατόμων ηλικίας μεγαλύτερης των 65 ετών. Η παρουσία της κΜ αυξάνει σημαντικά τη θνητότητα και τη θνησιμότητα, ενώ παράλληλα επηρεάζει και την ποιότητα ζωής. Με δεδομένη τη βαρύτητα της επίδρασης της κΜ στην υγεία, το ενδιαφέρον έχει στραφεί στην καλύτερη κατανόηση της υποκείμενης παθοφυσιολογίας της συχνής αυτής αρρυθμίας, σαν μια προσπάθεια να ανακαλυφθούν καινούργιες θεραπευτικές μέθοδοι. Πράγματι, η ανάπτυξη της κΜ οδηγεί σε ποικίλες μορφολογικές και λειτουργικές αλλαγές στον κόλπο, ο οποίος αναπτύσσει την αρρυθμία, μια διαδικασία γνωστή ως ηλεκτρική αναδιαμόρφωση. Επίσης, η κΜ εμπλέκεται σε μια προθρομβωτική ή υπερπηκτική κατάσταση, η οποία πιθανώς να συμβάλλει στον κίνδυνο θρομβοεμβολικών επεισοδίων.
Επιπρόσθετα, παρά τη χρήση νέων φαρμάκων και συσκευών, καθώς και την εφαρμογή στρατηγικών για τον περιορισμό της συχνότητάς της, η κΜ παραμένει κοινή και συχνή στην καθημέρα πράξη. Πρόσφατα, το ενδιαφέρον έχει στραφεί στην ανεύρεση του πιθανού ερεθίσματος ή της πιθανής πορείας, που οδηγούν στην αύξηση του κινδύνου ανάπτυξης και επανεμφάνισης της κΜ και των επιπλοκών της, ειδικά της θρομβοεμβολής. Οι κυττοκίνες της φλεγμονής έχουν προταθεί σαν πιθανοί υποκινητές της. Μια απόδειξη, η οποία στηρίζει την σχέση ανάμεσα στην κΜ και τη φλεγμονή, προκύπτει από τη συχνή συσχέτιση της κΜ με τις φλεγμονώδεις παθήσεις της καρδιάς, όπως η μυοκαρδίτιδα και η περικαρδίτιδα... Έχοντας υπ’ όψιν την επίπτωση σε περισσότερους από 2 εκατομμύρια Αμερικανούς (στην Ευρώπη δεν υπάρχουν σαφή στατιστικά στοιχεία), η κολπική μαρμαρυγή (κΜ) αποτελεί την πλέον κοινή αρρυθμία που αντιμετωπίζεται στην κλινική πράξη. Είναι συνήθως μια χρόνια, επαναλαμβανόμενη πάθηση που συνοδεύεται από σημαντική νοσηρότητα και θνησιμότητα. Τα προηγούμενα λίγα χρόνια έχουν δειχθεί σημαντικές εξελίξεις στην τεχνολογία και τις επεμβατικές τεχνικές, έτσι ώστε οι καρδιολόγοι να έχουν τη δυνατότητα να εξετάσουν 2 προσεγγίσεις αναφορικά με τη θεραπεία της κΜ:
• Φαρμακολογική θεραπεία – αυτή έχει περιορισμούς, και η πλέον κατάλληλη στρατηγική αντιμετώπισης ακόμα αποτελεί θέμα συζήτησης στη βιβλιογραφία.
• Τεχνικές κατάλυσης δια μέσου καθετήρων (RF catheter ablation) – οι τεχνικές αυτές έχουν αναπτυχθεί κατά τη διάρκεια της τελευταίας δεκαετίας με σκοπό την παρεμπόδιση της κΜ, και έχουν υιοθετηθεί ευρέως στα τριτογενή κέντρα περίθαλψης.
Υψηλά ποσοστά επιτυχίας έχουν αναφερθεί με τις τεχνικές κατάλυσης δια μέσου καθετήρων, και πρόσφατα έχει τεθεί το ερώτημα του κατά πόσο θα πρέπει να προτείνεται στους ασθενείς ως θεραπεία πρώτης επιλογής για την κΜ. Σε αυτήν την ανασκόπηση, θα συζητήσουμε τα θέματα που περιλαμβάνουν τις διαφορετικές στρατηγικές για τη θεραπεία της κΜ με ιδιαίτερη έμφαση στους κινδύνους και τα οφέλη της κατάλυσης δια μέσου καθετήρων, όπως και τον τρέχοντα ρόλο της κατάλυσης δια μέσου καθετήρων στην αντιμετώπιση των ασθενών με κΜ.

Ανακλήσεις Συσκευών
Στις 24 Μαΐου του 2005, ένας δημοσιογράφος δημοσίευσε στο New York Times και όχι σε κάποιο επιστημονικό περιοδικό, τις πιο ενδιαφέρουσες μελέτες στον τομέα της θεραπείας με συσκευές για το 2005. Το άρθρο ανέφερε ότι ένας ασθενής ηλικίας 21 ετών με υπερτροφική μυοκαρδιοπάθεια και εμφυτευμένο διαφλέβιο απινιδωτή (ICD) πέθανε αιφνιδίως. Η ανάλυση του ICD (Guidant 1861 Ventak Prizm 2, Guidant, Indinapolis, Indiana) κατέδειξε μια καταστροφική βλάβη της συσκευής. Όταν η συσκευή προσπάθησε να δώσει ηλεκτρική ενέργεια (shock) σε μια κοιλιακή ταχυ-αρρυθμία, έγινε μια ηλεκτρική εκκένωση στη μονωτική βάση της συσκευής, με αποτέλεσμα την δημιουργία βραχυκυκλώματος, το οποίο οδήγησε σε εκτεταμένη καταστροφή στη γεννήτρια ολόκληρου του κυκλώματος με αποτέλεσμα να μην δοθεί ηλεκτρική ενέργεια στον ασθενή, η οποία θα θεράπευε τη θανατηφόρο κοιλιακή αρρυθμία. Η μόνιμη καρδιακή βηματοδότηση αποτελεί την οριστική θεραπεία για τη συμπτωματική βραδυκαρδία από το 1959. Ωστόσο, δεδομένου ότι η τεχνολογία των συσκευών έχει εξελιχθεί, ειδικότερα με την ανάπτυξη της διπλοεστιακής βηματοδότησης και, τελευταία, με την αμφικοιλιακή βηματοδότηση, η βέλτιστη επιλογή της συσκευής και του προγραμματισμού έχει γίνει πιο περίπλοκη. Κολποκοιλιακή Διαδοχική Βηματοδότηση έναντι της Βηματοδότησης της Κορυφής της Δεξιάς Κοιλίας
Από τις δημιουργικές παρατηρήσεις του Carl Wiggers το 1932, οι ασχολούμενοι με τη φυσιολογία της καρδιάς είναι ενήμεροι ότι η έκτοπη κοιλιακή εκπόλωση οδηγεί σε ασύγχρονη συστολή και μειωμένη καρδιακή παροχή.Η πρώιμη κοιλιακή εκπόλωση οδηγεί σε κολποκοιλιακό (κ-Κ), ενδο-κοιλιακό και δια-κοιλιακό δυσυγχρονισμό και συνολικά ασύγχρονη συστολή. Στην προηγούμενη δεκαετία, διάφορες κλινικές μελέτες έχουν εστιαστεί στην πλέον ενστικτώδη ίσως από αυτές τις δυσπροσαρμοστικές αλλαγές: τον κ-Κ δυσυγχρονισμό. Σε 3 μεγάλες πολυκεντρικές κλινικές μελέτες, τη μελέτη Επιλογής του Τρόπου Βηματοδόσησης (MOST), την Καναδική Μελέτη Φυσιολογικής Βηματοδότησης (CTOPP), και τη μελέτη του Ηνωμένου Βασιλείου, Βηματοδότηση και Καρδιαγγειακά Συμβάματα (UKPACE), όπου σε κάθε μια τυχαιοποιήθηκαν πάνω από 2000 ασθενείς με συμπτωματική βραδυκαρδία, εξέτασαν την υπόθεση ότι η κ-Κ διαδοχική βηματοδότηση (DDD) ήταν ανώτερη (υπερείχε) από τη βηματοδότηση της κορυφής της δεξιάς κοιλίας (VVI)... Η κολπική μαρμαρυγή (κΜ) αποτελεί την πλέον συνήθη κλινική αρρυθμία, με 2,2 εκατομμύρια ανθρώπους στις ΗΠΑ και 4,5 εκατομμύρια στην Ευρώπη να επηρεάζονται από αυτή τη διαταραχή. Επιπλέον, η επίπτωσή της αυξάνεται λόγω γήρανσης του πληθυσμού, αυξημένης επίπτωσης της χρόνιας καρδιοπάθειας και της συχνής διάγνωσης της κΜ. Τα τελευταία 20 χρόνια οι εισαγωγές στα νοσοκομεία λόγω κΜ έχουν αυξηθεί κατά 66% και υπολογίζεται στις μέρες μας ότι στις ΗΠΑ η κΜ ευθύνεται για περίπου το 1/3 των εισαγωγών στα νοσοκομεία για διαταραχές του ρυθμού. Το ετήσιο χρηματικό κόστος για τη θεραπεία της νόσου είναι περίπου 3.600$ ανά ασθενή, κατατάσσοντας έτσι την κΜ στα πλέον δαπανηρά προβλήματα υγείας. Αν και δεν είναι απ’ ευθείας θανατηφόρα, η κΜ αυξάνει σημαντικά την ολική θνητότητα, κυρίως από καρδιαγγειακά αίτια όπως καρδιακή ανεπάρκεια και, πρωτίστως, από αγγειακά εγκεφαλικά επεισόδια.

Η χρήση των εμφυτεύσιμων απινιδωτών (ICDs) για την πρωτογενή πρόληψη του αιφνιδίου καρδιακού θανάτου (ΑΚΘ) έχει αποδειχθεί ότι μειώνει σημαντικά τη θνητότητα σε ασθενείς υψηλού κινδύνου, συμπεριλαμβανομένων των ασθενών μετά από έμφραγμα του μυοκαρδίου (ΕΜ) με δυσλειτουργία της αριστερής κοιλίας (ΑΚ) και των ασθενών με μη-ισχαιμική μυοκαρδιοπάθεια. Τα συντριπτικά στοιχεία υπέρ των ICDs για την πρωτογενή πρόληψη σε αυτό τον πληθυσμό των ασθενών έχουν οδηγήσει στις σημαντικές αλλαγές τόσο στην κλινική πρακτική όσο και στην ασφαλιστική κάλυψη/αποζημίωση κατά τη διάρκεια των προηγούμενων αρκετών ετών. Η πρόοδος στη φαρμακευτική θεραπεία της χρόνιας καρδιακής ανεπάρκειας οδήγησε σε αξιοσημείωτη βελτίωση της επιβίωσης και της ποιότητας ζωής. Παρόλα αυτά, ο κίνδυνος του αιφνίδιου καρδιακού θανάτου (SCD) παραμένει υψηλός στο γηράσκοντα πληθυσμό. Πρόσφατες εκτιμήσεις της ετήσιας επίπτωσης των καρδιακών θανάτων στις ΗΠΑ κυμαίνονται μεταξύ των 184.000 και 400.000 περιπτώσεων. Οι τυχαιοποιημένες κλινικές μελέτες δεν έχουν δείξει πειστικά ότι η αντιαρρυθμική θεραπεία μπορεί να μειώσει αποτελεσματικά τη θνητότητα σε ασθενείς με καρδιακή ανεπάρκεια. Δυστυχώς, η προσπάθεια ταυτοποίησης πληθυσμών υψηλού κινδύνου είχε πενιχρά αποτελέσματα ως προς τη μείωση της συνολικής επίπτωσης του SCD γενικά στον πληθυσμό, επειδή η πλειοψηφία των θανάτων αυτών συνέβαιναν σε ασθενείς, οι οποίοι δεν είχαν ταυτοποιήσιμους παράγοντες κινδύνου. Ο κίνδυνος SCD κυμαίνεται ανάμεσα στο 20% και στο 30% μεταξύ των ασθενών με μειωμένη συστολική λειτουργία της αριστερής κοιλίας. Επιπρόσθετα, ο κίνδυνος για SCD αυξάνει σχεδόν εκθετικά, όταν το ΚΕ μειώνεται <30%.
Παράλληλα με τη βαρύτητα της δυσλειτουργίας της αριστερής κοιλίας, ο βαθμός της λειτουργικής βλάβης, σύμφωνα με τη λειτουργική ταξινόμηση του New York Heart Association (NYHA) φαίνεται επίσης ότι είναι ένας ισχυρός ανεξάρτητος προγνωστικός παράγοντας του SCD. Παρά το γεγονός ότι ο απόλυτος αριθμός των SCD είναι μεγαλύτερος στους ασθενείς με class IV κατά NYHA, ο SCD ανέρχεται μόλις στο 35% της συνολικής θνητότητας στη συγκεκριμένη ομάδα ασθενών... Η κολπική μαρμαρυγή (κΜ) είναι η πιο συχνή εμμένουσα αρρυθμία στο γενικό πληθυσμό. Τα δεδομένα πρόσφατων μεγάλων πολυκεντρικών μελετών έχουν δείξει ότι οι θεραπευτικές στρατηγικές που βασίζονται στα αντιαρρυθμικά φάρμακα για τη διατήρηση του φλεβοκομβικού ρυθμού σε ασθενείς με παροξυσμούς κΜ έχουν σημαντικούς περιορισμούς. Επίσης, η επεμβατική αντιμετώπιση της αρρυθμίας αυτής με κατάλυση δεν είναι πιθανόν να αποτελέσει λύση εφικτή για το μεγαλύτερο μέρος των ασθενών, ιδιαίτερα των ηλικιωμένων, στο άμεσο μέλλον. Επομένως, εναλλακτικές θεραπείες θα πρέπει να αξιολογηθούν και ενδεχόμενα να εφαρμοστούν. Ο σκοπός αυτής της ανασκόπησης είναι να παρουσιάσει συνοπτικά τα μέχρι σήμερα δεδομένα σε σχέση με τη συμβολή της καρδιακής βηματοδότησης στην πρόληψη της κΜ.
Τύπος βηματοδότησης και κολπική μαρμαρυγή
Tα δεδομένα από ένα μεγάλο αριθμό παλαιότερων μη τυχαιοποιημένων μελετών παρατήρησης έχουν σαφώς δείξει ότι η κοιλιακή βηματοδότηση σχετίζεται με αυξημένο κίνδυνο κΜ σε σχέση με την κολπική βηματοδότηση. Οι μελέτες αυτές αποτέλεσαν την αφορμή για να σχεδιασθούν στη συνέχεια πολυκεντρικές προοπτικές κλινικές δοκιμές που συνέκριναν την κολπική βηματοδότηση (είτε AAI(R) είτε DDDR) με τον VVI (R) τύπο βηματοδότησης. Μια μετανάλυση των μελετών αυτών που συμπεριέλαβε 35,000 ασθενείς-έτη παρακολούθησης, έδειξε ότι η κολπική βηματοδότηση ήταν καλύτερη από την κοιλιακή βηματοδότηση στην πρόληψη της κΜ (λόγος κινδύνου-Hazard Ratio-HR 0.80, 95% όρια αξιοπιστίας-Confidence Interval-CI 0.72-0.89, P=.00003). Από τα ευρήματα της μελέτης αυτής φάνηκε επίσης πως το μεγαλύτερο όφελος της κολπικής βηματοδότησης αφορούσε στους ασθενείς με νόσο του φλεβοκόμβου... (excerpt)

H δυναμική ηχωκαρδιογραφία με χορήγηση δοβουταμίνης είναι μια αξιόπιστη και ασφαλής μέθοδος για τη διάγνωση και τον καθορισμό της θεραπευτικής στρατηγικής των ασθενών με γνωστή ή ύποπτη στεφανιαία νόσο. Όσον αφορά τα μείζονα συμβάματα κατά τη διενέργεια της μελέτης, κυρίως το έμφραγμα του μυοκαρδίου και την εμμένουσα κοιλιακή ταχυκαρδία, οι συχνότητες ήταν εξαιρετικά χαμηλές στις διάφορες δημοσιευθείσες σειρές. Ειδικά σε σχέση με την εμμένουσα κοιλιακή ταχυκαρδία, στη σειρά του Picano με 2800 ασθενείς έλαβαν χώρα 3 (0.1%) επεισόδια, ενώ στη σειρά των Secknus και Marwick με 3011 ασθενείς 5 (0.1%) επεισόδια.
Η κλινική σημασία της εμμένουσας κοιλιακής ταχυκαρδίας κατά τη διάρκεια της δυναμικής ηχωκαρδιογραφίας με δοβουταμίνη δεν είναι απολύτως γνωστή μέχρι σήμερα, κυρίως επειδή η συχνότητά της είναι πολύ μικρή, ενώ φαίνεται ότι η αιτιολογία της είναι ποικίλη. Είναι χαρακτηριστικό ότι κατά τη διάρκεια της δυναμικής ηχωκαρδιογραφίας με δοβουταμίνη μπορεί να προκληθούν όλες οι εμμένουσες μορφές κοιλιακού ρυθμού, δηλαδή επιταχυνόμενος ιδιοκοιλιακός ρυθμός, πολύμορφη κοιλιακή ταχυκαρδία και μονόμορφη κοιλιακή ταχυκαρδία... Αντιμετώπιση της Κολπικής Μαρμαρυγής σε Ασθενείς με Καρδιακή Ανεπάρκεια

Περίπου 5 εκατομμύρια ασθενείς στις ΗΠΑ υποφέρουν από καρδιακή ανεπάρκεια. Από αυτούς οι μισοί έχουν επηρεασμένη συστολική λειτουργικότητα της αριστερής κοιλίας και το 40 με 50% πάσχουν από καρδιακή ανεπάρκεια χωρίς επηρεασμό της συσταλτικότητας της αριστερής κοιλίας, πράγμα που συχνά συνδυάζεται με υπέρταση και την προχωρημένη ηλικία. Η επίπτωση της κολπικής μαρμαρυγής αυξάνει παράλληλα με τη βαρύτητα της συστολικού τύπου καρδιακής ανεπάρκειας και κυμαίνεται από 6% στους ασθενείς με ήπια καρδιακή ανεπάρκεια μέχρι >40% σε αυτούς με προχωρημένη καρδιακή ανεπάρκεια. Η κολπική μαρμαρυγή (κΜ) μπορεί να είναι ακόμα συχνότερη στους ασθενείς με καρδιακή ανεπάρκεια με καλή συσταλτικότητα και έχει βρεθεί περίπου στο 40% σε μεγάλες επιδημιολογικές μελέτες. Η έναρξη κΜ συνδυάζεται με επιδείνωση της καρδιακής ανεπάρκειας. Η απώλεια της κολπικής συστολής, η πολύ αργή ή πολύ γρήγορη κοιλιακή ανταπόκριση και η διακύμανση στο χρόνο πλήρωσης της καρδιάς μπορεί να επηρεάσουν αρνητικά την αιμοδυναμική κατάσταση του ασθενούς και να αυξήσουν τον τόνο του συμπαθητικού νευρικού συστήματος. Η κΜ συνδυάζεται με περισσότερες εισαγωγές στο νοσοκομείο και αυξημένη θνητότητα. Το αν η κΜ επιδεινώνει την καρδιακή ανεπάρκεια ή αν είναι δείκτης βαρύτητας της νόσου δεν έχει αποσαφηνιστεί πλήρως. Η θεραπεία του καρδιακού επανασυγχρονισμού (CRT) έχει καθιερωθεί σαν μέθοδος θεραπείας ασθενών με σοβαρού βαθμού συστολική δυσλειτουργία της αριστερής κοιλίας (ΑΚ), ευρύ QRS και κλινική εικόνα καρδιακής ανεπάρκειας (ΚΑ) ανθεκτικής στη φαρμακευτική αγωγη. Σκοπός της είναι ο επανασυγχρονισμός της κοιλιακής εκπόλωσης και της κοιλιακής μηχανικής συστολής.
Η ηχoκαρδιογραφία, έχει καθιερωθεί ως μια χρησιμη μέθοδος για την επιλογή ασθενών, πιθανών υποψηφίων για CRT καθώς και για την εκτίμηση του θεραπευτικού αποτελέσματος. Για το σκοπό αυτό χρησιμοποιούνται διάφορες τεχνικές όπως το Μ-mode, το παλμικό Doppler (PW), το συνεχές Doppler (CW), και τέλος το ιστικό Doppler (TDI).

Η κατεχολαμινεργική πολύμορφη κοιλιακή ταχυκαρδία (catecholaminergic polymorphic ventricular tachycardia-CPVT) αναφέρθηκε για πρώτη φορά από τον Reid και τους συνεργάτες του το 1975 και στη συνέχεια από τον Coumel και τους συνεργάτες του το 1978. Περιγράφηκε σαν μια οικογενής διαταραχή σε ασθενείς με δομικά υγιή καρδιά που χαρακτηρίζεται από πρόκληση κατά την άσκηση ή τη συγκίνηση κοιλιακών αρρυθμιών με χαρακτηριστική μορφολογία που είναι δυνατόν να καταλήξουν σε συγκοπή ή ακόμα και σε αιφνίδιο θάνατο. Από τότε που οι πρώτες γονιδιακές μεταλλάξεις του υποδοχέα της ρυανοδίνης ταυτοποιήθηκαν το 2001, εδραιώθηκε η άποψη ότι η CPVT προκαλείται από ανεξέλεκτη ενδοκυττάρια απελευθέρωση Ca2+ από το σαρκοπλασματικό δίκτυο. Μετέπειτα πειραματικές μελέτες κατέδειξαν ότι τέτοιες διαταραχές στη διακίνηση του ενδοκυττάριου Ca2+ προκαλούν αρρυθμίες μέσω μηχανισμών όψιμων μεταδυναμικών και πυροδοτούμενης δραστηριότητας. Η αμιωδαρόνη είναι ιωδιούχο παράγωγο του βενζοφουρανίου που παρουσιάζει αντιαρρυθμικές δράσεις των ομάδων Ι, ΙΙ, ΙΙΙ και IV. Η χορηγούμενη από του στόματος αμιωδαρόνη έχει πάρει έγκριση από τον FDA για τη θεραπεία σοβαρών, υποτροπιαζουσών, απειλητικών για τη ζωή αρρυθμιών, όπως είναι η κοιλιακή μαρμαρυγή (VF) ή η κοιλιακή ταχυκαρδία (VT), που συνοδεύονται από αιμοδυναμική αστάθεια. Οι καινούριες θεραπείες για την καρδιακή ανεπάρκεια που έχουν εισαχθεί τα τελευταία χρόνια έχουν βελτιώσει σημαντικά το προσδόκιμο επιβίωσης και έχουν μειώσει τον αριθμό των επανεισαγωγών στο νοσοκομείο λόγω οξείας απορρύθμισης. Παρά ταύτα ο ολικός αριθμός των εισαγωγών για καρδιακή ανεπάρκεια συνεχίζει να αυξάνει λόγω του προοδευτικά αυξανόμενου επιπολασμού της νόσου. Η πνευμονική συμφόρηση, που οφείλεται στην αυξημένη πίεση πλήρωσης του αριστερού κόλπου και της αριστεράς κοιλίας είναι το πλέον σύνηθες αίτιο που οδηγεί σε νοσηλεία.
Στις ΗΠΑ και σε άλλες αναπτυγμένες χώρες, η καρδιακή ανεπάρκεια απορροφά μεγάλο μερίδιο του προϋπολογισμού για την υγεία και περίπου το 70% αυτού του μεριδίου δαπανάται για τη θεραπεία της οξείας απορρύθμισης χρόνιας καρδιακής ανεπά-ρκειας. Για αυτό το λόγο είναι αναγκαία η τακτική παρακολούθηση των ασθενών αυτών στο εξωτερικό ιατρείο καρδιακής ανεπάρκειας, χωρίς όμως να έχει αποδειχθεί ότι με αυτό τον τρόπο μειώνεται η θνητότητα... Οι ταχυαρρυθμίες και οι βραδυαρρυθμίες κατά τη διάρκεια της κύησης μπορεί να θέσουν σε κίνδυνο αμφοτέρους μητέρα και έμβρυο. Ευτυχώς οι επικίνδυνες για τη ζωή διαταραχές ρυθμού σε ασθενείς που βρίσκονται σε ηλικία κύησης είναι πολύ σπάνιες. Οι εμμένουσες συμπτωματικές βραδυαρρυθμίες κατά την κύηση είναι επίσης ασυνήθεις και συχνά διαγιγνώσκονται προτού η ασθενής φτάσει στην ηλικία αυτή. Η εκτίμηση και θεραπεία των διαταραχών ρυθμού στην κυοφορούσα ασθενή απαιτεί προσοχή στην εκτίμηση των αιμοδυναμικών αλλαγών που συμβαίνουν κατά την κύηση και τη δράση των αντιαρρυθμικών φαρμάκων στο έμβρυο. Η Σχέση Μεταξύ Κολπικής Μαρμαρυγής και Κολπικού Πτερυγισμού. Μηχανισμοί και Κλινικές Εφαρμογές

Η ύπαρξη στενής σχέσης μεταξύ κολπικής μαρμα-ρυγής (ΚΜ) και κολπικού πτερυγισμού (ΚΠ) έχει από καιρό αναγνωριστεί. Ετσι, ασθενείς που αρχικά έχουν εκδηλώσει ΚΜ εκδηλώνουν επίσης ΚΠ και αντίστροφα ενώ συχνά οι δύο αυτές αρρυθμίες συνυπάρχουν στον ίδιο ασθενή μετά από καρδιοχειρουργική επέμβαση. Επίσης τα αντιαρρυθμικά κατηγορίας ΙΑ και ΙC όπως και η αμιωδαρόνη που χρησιμοποιούνται κατά της ΚΜ μπορούν να προκαλέσουν εμμένοντα ΚΠ. Αυτές οι συσχετίσεις δεν είναι συμπτωματικές αλλά αντανακλούν τη στενή σχέση μεταξύ των δύο αυτών αρρυθμιών όσον αφορά στην παθοφυσιολογία τους. Παρακάτω συνοψίζουμε τα στοιχεία που αναδεικνύουν αυτή τη σχέση και δίνουμε έμφαση στην κλινική σημασία τους.
Ρόλο κλειδί σε αυτή τη σχέση είναι το γεγονός ότι ο κλασσικός ΚΠ είναι ρυθμός μακρο – επανει-σόδου, όπου το κύμα (επανεισόδου) κινείται προς τα πάνω προς το μεσοκολπικό διάφραγμα και προς τα κάτω προς το ελεύθερο τοίχωμα του αριστερού κόλπου (RA) και αντίστροφα. Το κύκλωμα επανεισόδου περιλαμβάνει τον κοιλο-τριγλωχινικό (cavotri-cuspid) ισθμό (CTI) και σχεδόν πάντα ξεκινά σαν KM ποικίλης διάρκειας. Φαίνεται ότι η ανάπτυξη ΚΜ είναι προαπαιτούμενο για την εμφάνιση στη συνέχεια ΚΠ αφού κατά τη διάρκεια της ΚΜ δημιουργείται μια λειτουργική ζώνη αποκλεισμού μεταξύ των κοίλων φλεβών, οπότε προλαμβάνεται η δημιουργία μικρο – κυκλώματος επανεισόδου τοπικά. Έτσι, η ύπαρξη αυτής της ζώνης αποκλεισμού καθορίζει αν θα δημιουργηθεί εμμένουσα ΚΜ ή ΚΠ. Φαίνεται ότι στη συντριπτική πλειοψηφία των περιπτώσεων χωρίς προ-ϋπάρχουσα ΚΜ, δεν μπορεί να υπάρξει ΚΠ εκτός εάν η ζώνη αυτή προϋπάρχει για ανατομικούς λόγους. Σε αυτή την περίπτωση η προϋπάρχουσα ΚΜ δεν είναι απαραίτητη για δημιουργία ΚΠ... (excerpt)

Η κολπική μαρμαρυγή είναι η πιο κοινή καρδιακή αρρυθμία και είναι υπεύθυνη για το 1/3 περίπου των εισαγωγών στα νοσοκομεία λόγω διαταραχών του καρδιακού ρυθμού. Σχετίζεται με αυξημένο κίνδυνο εγκεφαλικού επεισοδίου, καρδιακής ανεπάρκειας καθώς και με αυξημένη ολική θνησιμότητα. Η τεράστια πρόοδος στα αποτελέσματα της κατάλυσης με χρήση καθετήρων κατά την τελευταία δεκαετία έχει δώσει την εντύπωση πως η θεραπεία αυτή μπορεί να αποτελεί πρώτης γραμμής επιλογή για την αντιμετώπιση της αρρυθμίας. Παρόλα αυτά, υπάρχει ακόμη χώρος για περαιτέρω βελτίωση πριν η κατάλυση αποτελέσει οριστικά την πρώτης γραμμής θεραπεία ιδιαίτερα στις περιπτώσεις ασθενών με χρόνια κολπική μαρμαρυγή. Πως φθάσαμε λοιπόν μέχρι εδώ, που βρισκόμαστε σήμερα και πως προδιαγράφεται το μέλλον για την συγκεκριμένη αυτή θεραπευτική στρατηγική;

Η μεταβολή του μήκους κύκλου της κΜ ως δείκτης επιτυχούς κατάλυσης.
Πειραματικές μελέτες χαρτογράφησης έχουν δείξει ότι το μήκος κύκλου της κΜ συσχετίζεται με τη διάρκεια των τοπικών ανερέθιστων περιόδων, μειώνεται όσο παραμένει η αρρυθμία και παρατείνεται προοδευτικά ή απότομα αμέσως πριν αναταχθεί η αρρυθμία με τη χορήγηση φαρμάκων. Εχει επίσης δειχθεί ότι το μήκος κύκλου της κΜ όπως μετράται στην περιοχή του στεφανιαίου κόλπου, επιμηκύνεται προοδευτικά μετά από επιτυχή απομόνωση των πνευμονικών φλεβών και ανάταξη της αρρυθμίας με κατάλυση. Επιπλέον, όσο αυξάνεται ο αριθμός των βλαβών στον αριστερό κόλπο τόσο αυξάνεται και το μήκος κύκλου της κΜ και το μεγαλύτερο μήκος κύκλου παρατηρείται σε περιπτώσεις συνδυασμένης κατάλυσης των πνευμονικών φλεβών, του ωτίου του αριστερού κόλπου και της περιοχής του αριστερού κόλπου που έρχεται σε επαφή με τον στεφανιαίο κόλπο. Όταν επιτευχθεί μήκος κύκλου 180-200 ms, τότε η κΜ είτε ανατάσσεται αυτόματα σε φλεβοκομβικό ρυθμό είτε, στην πλειοψηφία των περιπτώσεων, μεταπίπτει σε κολπική ταχυκαρδία η οποία μπορεί να χαρτογραφηθεί και να καταλυθεί με τις συμβατικές μεθόδους. Στο 80% περίπου των ασθενών, το μήκος κύκλου αυξάνεται παράλληλα και στους δύο κόλπους πράγμα που υποδηλώνει πως ο αριστερός κόλπος είναι αυτός που επιβάλλει τον ρυθμό και τη συχνότητα. Αντίθετα, στο 20% των περιπτώσεων, το μήκος κύκλου του δεξιού κόλπου είναι μικρότερο απο αυτό του αριστερού κόλπου όπως μετράται στην περιοχή του αριστερού ωτίου. Στις περιπτώσεις αυτές η κατάλυση μέσα στο δεξιό κόλπο ανατάσσει την κΜ πράγμα που σημαίνει πως ο δεξιός κόλπος είναι υπεύθυνος για την αρρυθμία... (excerpt)

Βελτιστοποίηση του Καρδιακού Ανασυγχρονισμού

Εισαγωγή
Η θεραπεία του καρδιακού επανασυγχρονισμού (CRT) ελαττώνει τη θνητότητα και νοσηρότητα στους ασθενείς με καρδιακή ανεπάρκεια. Η βελτιστοποίηση της θεραπείας αυτής με τη ρύθμιση δυο βασικών βηματοδοτικών παραμέτρων, της διακοιλιακής και κολποκοιλιακής (κ-Κ) καθυστέρησης, παρότι συμπεριλαμβάνεται σε σημαντικές κλινικές μελέτες παραμένει ζήτημα σε εκκρεμότητα. Πρόσφατες κατευθυντήριες οδηγίες από την Αμερικανική εταιρεία υπερηχοκαρδιογραφίας συστήνουν βελτιστοποίηση του κ-Κ συγχρονισμού. Στην παρούσα ανασκόπηση παρουσιάζουμε αντικειμενικά τα στοιχεία υπέρ της συγκεκριμένης άποψης. Γιατί να βελτιστοποιήσουμε την CRT; Πώς; Πότε; Και το κυριότερο: Βοηθά αυτό τους ασθενείς;
Ερευνητική στρατηγική
Στοιχεία αναζητήθηκαν στις βάσεις δεδομένων MEDLINE, EMBASE, PubMed και στη βιβλιοθήκη Cochrane μέχρι και το Νοέμβριο 2007 με λήμματα τίτλου και λέξεις κλειδιά ή συνδυασμούς αυτών. Ελέγχθησαν καταχωρήσεις εγκεκριμμένων μελετών από τη Διεθνή Επιτροπή των Εκδοτών Ιατρικών Περιοδικών καθώς και οι βιβλιογραφικές αναφορές των επιλεγμένων άρθρων, ενω εξετάσθηκαν ανακοινώσεις σε διεθνείς συναντήσεις από το 2000 ως το 2007. Κολπική Μαρμαρυγή και Σύνδρομο Brugada

Το σύνδρομο Brugada χαρακτηρίζεται από εικόνα RBBB, ανάσπαση του τμήματος ST στις απαγωγές V1-V3 του ΗΚΓ και από τάση για αιφνίδιο καρδιακό θάνατο. Πολλές μελέτες έχουν συνδέσει τη γενετική βάση του συνδρόμου με μεταλλάξεις σε γονίδια που κωδικοποιούν την α υπομονάδα του διαύλου Να+. Πιο πρόσφατες μελέτες το συσχετίζουν με μεταλλάξεις στα γονίδια των α και β υπομονάδων των διαύλων Ca2+ και στο γονίδιο του ομοιάζοντος στην αφυδρογονάση 1 της 3-φωσφορικής γλυκερόλης ενζύμου (glycerol-3-phosphate dehydrogenase1-like/GPD1L). Τα 4 αυτά γονίδια, από μακρού ταυτοποιηθέντα, έχουν εκτιμηθεί ως υπόλογα για το 28% περίπου του συνδρόμου Brugada. Έτσι, το 72% των περιπτώσεων παραμένει μη ταυτοποιημένο γονοτυπικά. Η κολπική μαρμαρυγή (κΜ) είναι η πιο συχνή αρρυθμία για την οποία αναζητάται ιατρική βοήθεια. Έχει επίπτωση περίπου 1% στον ενήλικο πληθυσμό των ΗΠΑ και καθώς ο πληθυσμός γηράσκει, η συχνότητα της κολπικής μαρμαρυγής θα γίνεται ολοένα και πιο συχνή. Στους περισσότερους ασθενείς με κολπική μαρμαρυγή η θεραπεία περιλαμβάνει τη χρήση αντιαρρυθμικών φαρμάκων (ΑΑΦ) για την ανάταξη της αρρυθμίας και τη διατήρηση του φλεβοκομβικού ρυθμού. Ωστόσο, η χρήση τους περιορίζεται από τη μέτρια αποτελεσματικότητα, από τα επεισόδια δυσανεξίας και φαρμακευτικής τοξικότητας, καθώς και από τη δυνητική προαρρυθμική τους δράση. Παράλληλα, οι τεχνικές κατάλυσης με υψίσυχνο ρεύμα (ablation) κερδίζουν συνεχώς έδαφος. Και εδώ όμως, η αποτελεσματικότητα είναι μεν αποδεκτή αλλά όχι πλήρης, ενώ προκύπτουν σημαντικά θέματα ασφάλειας. Ωστόσο, εάν οι τεχνικές κατάλυσης βελτιωθούν, τίθεται το ερώτημα εάν πλέον υπάρχει θέση για ΑΑΦ και εάν ναι πότε και ποια απ' αυτά θα πρέπει να χρησιμοποιηθούν.
Το μέγεθος του προβλήματος
Η νοσηρότητα και η θνητότητα της κολπικής μαρμαρυγής είναι σημαντική. Τα επεισόδια δύσπνοιας, αισθήματος παλμών, τα αγγειακά εγκεφαλικά επεισόδια (ΑΕΕ) και η απορρύθμιση της καρδιακής ανεπάρκειας προκαλούμενα από την κΜ είναι αιτία για συχνές ιατρικές επισκέψεις και επιπλέον νοσηλείες. Η αποτελεσματικότητα των σημερινών φαρμάκων για τη διατήρηση του φλεβοκομβικού ρυθμού είναι ανεπαρκής. Οι περισσότεροι ασθενείς με κΜ, σε απουσία ενός παροδικού εκλυτικού παράγοντα όπως πχ το χειρουργείο, θα έχουν υποτροπή της κΜ παρά τη χορηγούμενη αντιαρρυθμική αγωγή. Ακόμα, αρκετές μελέτες (πχ CAST, SWORD) με διαφορετικά ΑΑΦ κατέδειξαν αυξημένη θνητότητα με τη χρήση τους σε έδαφος οργανικής καρδιακής νόσου.
Η ανάγκη χρήσης φαρμάκων για την ανάταξη και τη διατήρηση του φλεβοκομβικού ρυθμού έχει αμφισβητηθεί από πολλούς, καθώς τα αποτελέσματα πρόσφατων μελετών όπως οι AFFIRM,RACE, PIAF, STAF, DIAMOND-AF,κ.λ. δεν αναδεικνύουν όφελος στη θνητότητα στοχεύοντας στη διατήρηση του ρυθμού. Από την άλλη και παρά την έλλειψη σκληρών δεδομένων υπέρ της διατήρησης του ρυθμού, πολλοί κλινικοί υποστηρίζουν ότι μια κΜ έστω και με ελεγμένη συχνότητα δεν είναι ισοδύναμη με το φυσιολογικό φλεβοκομβικό ρυθμό. Είναι πιθανό ο φλεβοκομβικός ρυθμός να πλεονεκτεί, αλλά η τοξικότητα και η προαρρυθμική δράση των σημερινών ΑΑΦ αντισταθμίζουν το οποιοδήποτε όφελος... (excerpt)

Κριτική Αξιολόγηση της Εμφύτευσης Απινιδωτή στην Πρόληψη του Αιφνιδίου Καρδιακού Θανάτου

Από τις πρώτες περιγραφές της εξωτερικής απινίδωσης στη δεκαετία του ’60 και την πρώτη εμφύτευση καρδιομετατροπέα-απινιδωτή (ICD) στον άνθρωπο το 1980 από τον Mirowski, η πρόληψη του αιφνιδίου καρδιακού θανάτου απομακρύνθηκε τόσο από την αντιαρρυθμική αγωγή όσο και από τις επεμβάσεις κατάλυσης. Μετά δε την ανακοίνωση της μελέτης AVID για τη δευτερογενή πρόληψη, η τοποθέτηση ICD δοκιμάσθηκε σε ευρύτερη κλίμακα και σε πληθυσμούς χαμηλότερου κινδύνου για πρωτογενή πρόληψη και αναβαθμίσθηκε από μια απονενοημένη λύση σε αναγνωρισμένη θεραπεία ευρείας εφαρμογής. Τα τελευταία 15 χρόνια ο ετήσιος ρυθμός εμφύτευσης απινιδωτών αυξήθηκε κατά 20 φορές.
Παρά το γεγονός ότι το τραγικό και απρόοπτο του αιφνιδίου θανάτου προκαλεί φόβο και αβεβαιότητα, οι συναισθηματικές περιπλοκές δεν θα πρέπει να παρεμβαίνουν στην αξιολόγηση των επιστημονικών δεδομένων. Η εν λόγω θεραπεία έχει εκτός του βασικού κόστους των περίπου 30000 $, και άλλες σημαντικές επιπτώσεις. Πρέπει να σταθμίζεται το όφελος από τις ζωές που σώζονται με τις επιπλοκές που σχετίζονται με τη χορήγηση αδόκιμων εκφορτίσεων, τα προβλήματα κατά την εμφύτευση, την πιθανή δυσλειτουργία της συσκευής, τις περιπτώσεις ανάκλησης συσκευών και τέλος τον κίνδυνο προαρρυθμίας.

Είναι ο Κίνδυνος για Κολπική Μαρμαρυγή Μεγαλύτερος στους Αθλητές από ότι στο Γενικό Πληθυσμό; Μια Συστηματική Ανασκόπηση και Μετα-ανάλυση

Η επίπτωση της κολπικής μαρμαρυγής (κΜ) στο γενικό πληθυσμό αυξάνεται προοδευτικά μετά την ηλικία των 40 ετών αλλά εμφανίζει απότομη άνοδο μετά την ηλικία των 65 ετών. Η επίπτωση της νόσου σε νεότερα άτομα είναι πολύ χαμηλή, ενώ είναι σχεδόν μηδενική σε ηλικίες κάτω των 25 ετών. Εντούτοις, σύμφωνα με αρκετές μελέτες παρατήρησης, η επίπτωση της κΜ αυξάνεται σε νέους αθλητές μετά από πολλά χρόνια προπόνησης. Η παρατήρηση αυτή έχει επανειλημμένα επιβεβαιωθεί σε μελέτες ασθενούς-μάρτυρα οι οποίες συνέκριναν αθλητές με άτομα από το γενικό πληθυσμό που δεν ήταν αθλητές.
Η κΜ αποτελεί, πιθανώς, την πιο συχνά εμφανιζόμενη ταχυαρρυθμία μεταξύ των αθλητών αλλά η αιτιολογία και η παθοφυσιολογία της στη συγκεκριμένη ομάδα ασθενών δεν είναι ακόμα επαρκώς κατανοητή. Η επιρρέπεια των αθλητών στην κΜ έχει αποδοθεί στην αύξηση του τόνου του παρασυμπαθητικού με τη συνεπακόλουθη βραδυκαρδία που μπορεί να οδηγήσει σε διασπορά της κολπικής επαναπόλωσης η οποία μπορεί να αυξήσει την πιθανότητα εμφάνισης κΜ. Άλλες μελέτες έχουν αποδώσει τη συχνή επίπτωση της κΜ στους αθλητές, στη διαδικασία αναδιαμόρφωσης (remodeling) και αύξησης των διαστάσεων του αριστερού κόλπου. Μια ανασκόπηση που δημοσιεύτηκε πρόσφατα τονίζει τη σημασία του θέματος αυτού και προτείνει τη διεξαγωγή μεγαλύτερων προοπτικών μελετών που θα αποσαφηνίσουν τη συσχέτιση κΜ και σωματικής αντοχής... Το σύνδρομο Brugada είναι μια κληρονομική πάθηση που σχετίζεται με σοβαρές καρδιακές αρρυθμίες και χαρακτηρίζεται ηλεκτροκαρδιογραφικά από ανάσπαση του ST διαστήματος με το κυρτό προς τα πάνω (coved type) και ανύψωση του σημείου J τουλάχιστον κατά 2mm, σε 2 τουλάχιστον δεξιές προκάρδιες απαγωγές του ηλεκτροκαρδιογραφήματος (V1 – V3). Οι ηλεκτροκαρδιογραφικές αυτές αλλαγές δεν σχετίζονται με ισχαιμία, ηλεκτρολυτικές διαταραχές, ή γνωστή δομική καρδιακή νόσο. Η νόσος έχει συσχετισθεί με κοιλιακές αρρυθμίες, κολπικές αρρυθμίες, συγκοπή, και αιφνίδιο θάνατο. Η διάγνωση απαιτεί την παρουσία των ηλεκτροκαρδιογραφικών μεταβολών που περιγράφονται παραπάνω (τύπος 1) μαζί με τουλάχιστον ένα από τα παρακάτω αναγνωρισμένα διαγνωστικά κριτήρια: συγκοπή, επεισόδιο ανακοπής στο παρελθόν, τεκμηριωμένη ή προκλητή πολύμορφη κοιλιακή ταχυκαρδία ή κοιλιακή μαρμαρυγή, οικογενειακό ιστορικό αιφνίδιου θανάτου σε ηλικία < 45 έτη, ή τον τύπο 1 του συνδρόμου Brugada και/ή νυκτερινή εργώδη αναπνοή. Είναι μια οικογενής κατάσταση η οποία μεταβιβάζεται με αυτοσωματικό επικρατούντα χαρακτήρα με ατελή διεισδυτικότητα και με επίπτωση 5 έως 66 περιπτώσεις ανά 10.000. Η πλειονότητα των περιπτώσεων με σύνδρομο Brugada προκαλείται από μεταλλάξεις στο γονίδιο του καρδιακού διαύλου των ιόντων νατρίου SCN5A, του γονιδίου που είναι υπεύθυνο για την α-υπομονάδα του διαύλου ιόντων νατρίου. Πρόσφατα όμως αναγνωρίστηκε μια ομάδα ασθενών με οικογενές σύνδρομο αιφνιδίου θανάτου, στους οποίους αναγνωρίστηκε ο φαινότυπος του συνδρόμου Brugada σε συνδυασμό με βραχέα διαστήματα QT (QTc≤360ms). Σε αυτούς τους ασθενείς βρέθηκαν μεταλλάξεις που οδήγησαν σε απώλεια λειτουργικότητας στα γονίδια CACNA1C (A39V και G490R) και CACNB2 (S481L), τα οποία κωδικοποιούν τις α1 και β2b υπομονάδες του L-τύπου διαύλων ασβεστίου, γεγονός το οποίο φαίνεται να είναι ο αιτιολογικός παράγοντας. Παρομοίως, μετάλλαξη στο γλυκερο-3-φωσφο αφυδρογονάση 1-ομοιάζον γονίδιο (GPD1-L) έχει αναγνωριστεί ως ένα λιγότερο κοινό αίτιο του συνδρόμου Brugada. Σε αυτούς τους ασθενείς, η μετάλλαξη στο GPD1-L γονίδιο προκαλεί μια μείωση στην έκφραση επί της μεμβράνης του SCN5A και συνεπώς μια μείωση στο δυναμικό του νατρίου της κυτταρικής μεμβράνης, περίπου στο 50%, που έχει ως αποτέλεσμα την εμφάνιση του φαινοτύπου του συνδρόμου Brugada... H κολπική μαρμαρυγή (κΜ) είναι μια εξαιρετικά συχνή αρρυθμία, της οποίας ο επιπολασμός αυξάνεται με την ηλικία. Η νοσηρότητα από την κΜ είναι σημαντική. Το κύριο πρόβλημα είναι η μείωση της λειτουργικής ικανότητας που προκαλείται από την εμφάνιση συμπτωμάτων, όπως αισθήματος παλμών, εύκολης κόπωσης και δύσπνοιας ή επιδείνωσης προϋπάρχουσας καρδιακής ανεπάρκειας ή στεφανιαίας νόσου. Συνεπώς, η διατήρηση του φλεβοκομβικού ρυθμού, αν και εφόσον επιτυγχάνεται, είναι επιθυμητή λαμβάνοντας υπόψιν αφενός τη σχέση κόστους – οφέλους και αφετέρου τις παρενέργειες των αντιαρρυθμικών φαρμάκων για τον ασθενή. H ηχωκαρδιογραφία είναι μια μέθοδος που μπορεί να προβλέψει ποιοι ασθενείς έχουν την τάση να εμφανίσουν κΜ. Σκοπός της μικρής αυτής ανασκόπησης είναι η παράθεση των πρόσφατων ηχωκαρδιογραφικών – Doppler δεδομένων που συνδέονται με τους παροξυσμούς κΜ.
Παραδοσιακά οι παράγοντες που συνδέονται με υποτροπή κΜ είναι:
• Σοβαρή διάταση του αριστερού κόλπου
• Συστολική δυσλειτουργία της αριστερής κοιλίας
• Παρουσία ρευματικής βλάβης της μιτροειδούς βαλβίδας
Όσον αφορά τη μη ρευματικής αιτιολογίας κΜ, φαίνεται ότι η εκτίμηση της διαστολικής λειτουργίας παίζει σημαντικό ρόλο στην πρόβλεψη των ασθενών που θα εμφανίσουν κΜ. Ιδίως σε ασθενείς άνω των 65 χρόνων φαίνεται ότι είναι δυνατή η πρόβλεψη της μετάπτωσης της παροξυσμικής κΜ σε χρόνια. Ο αριθμός των ασθενών που πάσχουν από ισχαιμική μυοκαρδιοπάθεια αυξάνεται σταθερά στο Δυτικό κόσμο, κυρίως λόγω της ανάπτυξης των φαρμακευτικών και επαναγγειωτικών τεχνικών, οι οποίες παρέχουν ένα βελτιωμένο τελικό αποτέλεσμα. Ωστόσο, παρόλη την πρόοδο που έχει σημειωθεί, οι ασθενείς παραμένουν σε υψηλό κίνδυνο για αιφνίδιο θάνατο. Η είσοδος του εμφυτεύσιμου απινιδωτή (ICD) είχε μια τεράστια επίδραση στην επιβίωση και κατά συνέπεια στη θεραπεία των ασθενών με ισχαιμική μυοκαρδιοπάθεια. Η μελέτη MADIT – II έδειξε ότι σε ασθενείς με ισχαιμική μυοκαρδιοπάθεια και κλάσμα εξωθήσεως αριστερής κοιλίας < 30%, η προφυλακτική εμφύτευση ICD είχε ως αποτέλεσμα την μείωση κατά 31% της θνητότητας ανεξαρτήτως αιτίας κατά την τριετή παρακολούθηση. Αφού επαληθεύθηκαν τα παραπάνω αποτελέσματα με τη μελέτη SCD-HeFT, η τοποθέτηση ICD εντάχθηκε ως ένδειξη IIα στις κατευθυντήριες οδηγίες των ACC/AHA/ESC. Σαν αποτέλεσμα παρατηρήθηκε μια ραγδαία αύξηση στην τοποθέτηση ICD, με την ανάλογη αύξηση του κόστους. Ωστόσο μόνο στο 35% των ασθενών ο ICD χρειάστηκε να ανατάξει κάποια επικίνδυνη αρρυθμία κατά την παρακολούθηση των πρώτων 3 ετών. Από τη στιγμή που η πλειονότητα των ασθενών δεν χρειάστηκε ηλεκτρική ανάταξη από τη συσκευή, ήταν αναγκαίος ένας επιπρόσθετος έλεγχος των κριτηρίων εμφύτευσης ώστε να επιλεγούν ασθενείς που ήταν πιθανότερο να ωφεληθούν από αυτή τη θεραπεία ... Τυχαιοποιημένες κλινικές μελέτες ανέδειξαν την ξεκάθαρη υπεροχή του διαφλέβιου εμφυτεύσιμου καρδιακού απινιδωτή (ICD) έναντι των αντιαρρυθμικών φαρμάκων ως πρωτογενή θεραπεία σε ασθενείς υψηλού κινδύνου για κοιλιακές αρρυθμίες καθώς και στη δευτερογενή πρόληψη. Κατά συνέπεια, η τοποθέτηση ICD είναι πλέον η θεραπεία πρώτης επιλογής στους ασθενείς με κίνδυνο να εμφανίσουν αυτές τις αρρυθμίες.
Ωστόσο, εκείνοι οι οποίοι φέρουν απινιδωτή μπορεί να χρειαστούν συμπληρωματική αγωγή με αντιαρρυθμικά φάρμακα (AAΦ) κάποια στιγμή στο μέλλον. Σε πολλές μελέτες αναφέρεται ότι το 16-17% των ασθενών στους οποίους έγινε εμφύτευση ICD κατέληξε τελικά να λαμβάνει συμπληρωματική αγωγή με AAΦ. Στη μελέτη MADIT-II το 10% των ασθενών με ICD έλαβε αγωγή με AAΦ στο τελευταίο follow-up (μέσος όρος 20 μήνες). Στη μελέτη AVID η οποία συνέκρινε τη χορήγηση αντιαρρυθμικών φαρμάκων με την εμφύτευση απινιδωτή, φάνηκε ότι το 18% των ασθενών που τυχαιοποιήθηκαν στο σκέλος του ICD, έλαβαν AAΦ μετά από περίοδο παρακολούθησης 135 ημερών. Δεδομένου ότι πρόκειται για ελεγχόμενες κλινικές μελέτες, η προσθήκη των AAΦ σε ασθενείς με ICD έγινε προσπάθεια να αποφευχθεί. Επομένως, το ως άνω ποσοστό αντιπροσωπεύει πιθανότατα την ελάχιστη ανάγκη χρήσης AAΦ σε συνδυασμό με ICD στα πλαίσια δευτερογενούς πρόληψης... (excerpt)

Η κολπική μαρμαρυγή (κΜ) αποτελεί ένα από τα σημαντικότερα συνυπάρχοντα νοσήματα ασθενών με καρδιακή ανεπάρκεια. Το συγκεκριμένο άρθρο παραθέτει στοιχεία που αφορούν τη σχέση της κΜ με τη θεραπεία του καρδιακού επανασυγχρονισμού. Μελέτες για τη χρήση καρδιακού επανασυγχρονι-σμού σε ασθενείς με καρδιακή ανεπάρκεια και κΜ αποδεικνύουν την αποτελεσματικότητά της, δεδομένου ότι μειώνεται η εμφάνιση της τελευταίας και επάγεται η αντίστροφη αναδιαμόρφωση του αριστερού κόλπου. Δεν υπάρχουν στοιχεία σχετικά με την κατάλυση της κΜ σε ασθενείς με καρδιακό επανασυγχρονισμό αλλά είναι πιθανό να αποτελεί θεραπευτική επιλογή. Εξάλλου οι σύγχρονοι εμφυτεύσιμοι βηματοδότες και καρδιομετατροπείς-απινιδωτές διαθέτουν αλγόριθμους για αποτελεσματι-κότερη διάγνωση και αντιμετώπιση της κΜ.
Στο N Engl J Med ο Braunwald όρισε την καρδιακή ανεπάρκεια (ΚΑ) και την κΜ ως τις επιδημίες της χιλιετηρίδας. Είναι βεβαίως γνωστό ότι η κΜ είναι η συχνότερη αρρυθμία σε ασθενείς με ΚΑ, υπάρχει μάλιστα γραμμική σχέση της συχνότητάς της με το λειτουργικό στάδιο κατά NYHA και συγκεκριμένα μπορεί να αγγίζει σχεδόν το 50% σε ασθενείς με NYHA class IV. Επιπλέον σχετίζεται με αυξημένη θνητότητα και επιδείνωση της ΚΑ σε ασθενείς με ασυμπτωματική και συμπτωματική συστολική δυσλειτουργία της αριστεράς κοιλίας. Δεδομένου ότι η κΜ αποτελεί μία από τις σημαντικότερες συνυπάρχουσες νόσους ασθενών με ΚΑ είναι προφανές το ενδιαφέρον μας να κατανοήσουμε παραμέτρους όπως η σχέση της με τη θεραπεία του καρδιακού επανασυγχρονισμού (CRT) που είναι μη-φαρμακευτική θεραπευτική επιλογή που όλο και συχνότερα εφαρμόζεται στη συνεχώς αυξανόμενη υποομάδα ασθενών με ΚΑ (NYHA ΙΙ-IV) και ενδοκοιλιακό δυσυγχρονισμό... Cardiac Resynchronization Therapy in Heart Failure: Patients with Narrow QRS

Heart failure remains a significant health problem in the Western countries despite the evolution achieved in terms of heart disease prevention and medical treatment. Its incidence and its prevalence reach 550,000/year and 5 million respectively in the United States. As a consequence, heart failure causes about 287,000 deaths in the US each year and puts a financial burden to the health system of $29.6 billion dollars due to the increasing number of hospitalizations especially in elderly patients.
Cardiac resynchronization therapy (CRT) is a relatively recent advance in the management of heart failure patients, which has been very hopeful since its development. Large trials have established the efficacy of CRT in improving functional capacity (MUSTIC, MIRACLE) and reducing mortality (COMPANION, CARE HF) in patients with advanced heart failure (NYHA III-IV), reduced ejection fraction (EF<35%) and wide QRS complex >120 - 130 ms (MIRACLE). Data from meta-analyses also confirm the beneficial effect of CRT with respect to mortality from worsening heart failure and all cause mortality in patients with wide QRS complex.
Nevetheless, not all patients suffering from heart failure have a prolonged QRS. On the contrary, it has been reported that nearly 50% of heart failure patients have a QRS < 120 ms. Moreover, CRT has been shown to improve haemodynamic variables in patients with heart failure and narrow QRS. Achilli et al, who studied 52 patients, reported that CRT produced similar clinical and functional benefit in patients with wide or narrow QRS, the cut-off point being 120 ms provided they had mechanical dyssynchrony. Bleeker at al came up with similar results when they compared 33 patients with normal QRS duration with an equal number of subjects with wide QRS. What is New in the ESC Guidelines for the Management of Atrial Fibrillation

The European Society of Cardiology (ESC) and the European Heart Rhythm Association (EHRA) have developed the 2010 Clinical Practice Guidelines covering atrial fibrillation (AF), the most common cardiac arrhythmia occurring in 1-2% of the general population. Advance has been made regarding the dynamic development of AF from a preclinical state to an irreversible cardiac arrhythmia and a novel classification of AF has been adopted based on the presentation and duration of the arrhythmia: first diagnosed, paroxysmal, persistent, long-standing persistent and permanent AF are the 5 types of AF in use for clinical management of patients with AF.
Structural and electrical remodelling are hallmarks of the pathophysiological changes facilitating the initiation and perpetuation of AF. While atrial fibrosis was the main cause of nonhomogeneity of conduction according to earlier ESC guidelines, nowadays any kind of structural abnormality (inflammatory changes, amyloid deposit, apoptosis, necrosis, hypertrophy, microvascular changes, etc.) is believed to trigger the electrical dissociation between muscle bundles and permit small re-entrant circuits to stabilize the arrhythmia. The adage ‘atrial fibrillation begets atrial fibrillation’ describes electrical remodelling due to shortening of atrial refractory period, which is attributed to down-regulation of the L-type Ca2+ inward current and up-regulation of inward rectifier K+ currents. Although the exact role of the genome in the pathogenesis of AF is not known, numerous inherited cardiac syndromes and mutations have lately been associated with AF and should be elucidated. Mutations in the gene coding for atrial natriuretic peptide, loss of function mutations in the cardiac sodium channel gene SCN5A or gain of function mutations in the cardiac potassium channel are related to familial AF and genetic loci close to the PITX2 and ZFHX3 genes are currently associated with enhanced risk for cardioembolic stroke... In recent years, there has been a remarkable increase in the number of patients who benefit from cardiovascular implantable devices. On the other hand, magnetic resonance imaging (MRI) compared with other imaging techniques undoubtedly has many advantages regarding its discrimination ability without radiation exposure. The risks of scanning device patients that have non “MR Safe” or “MRConditional” devices (Table 1), due to field effects, are well known as are the risk mitigation strategies.
Hazards and safety concerns
There are three distinct mechanisms associated with MRI that can give rise to potential risks related to cardiovascular devices: (1) the static magnetic field, (2) radiofrequency (RF) energy, (3) gradient magnetic fields or any combination of them. Static main magnetic field exposes ferromagnetic components to mechanical forces and torque and causes unpredictable magnetic sensor activation, reed switch closure and electrocardiograms distortions on electronic implantable pulse generators (IPGs), implantable cardioverter defibrillators (ICDs) and cardiac resynchronization therapy (CRT) devices. Modulated RF field interacts with pacemaker devices and leads causing heating of cardiac tissue adjacent to electrodes, reset and sensing problems of the device. ICDs can falsely detect RF field as ventricular tachycardia (VT) and start the sequence of defibrillation. Gradient magnetic field can induce currents in electrically conductive wires and leads that could cause over-/under-sensing and induce arrhythmias. Combined field effects can cause alteration of device function, vibrations, electronic reset and device or lead damage. Electrical reset is a safety emergency mode that limits pacemaker functionality to minimal in case of battery depletion or dip due to MR field interference. Factory default settings and usually a VVI mode are set while ICD therapies are deactivated. Because ICD’s parts, such as capacitors and batteries, are much larger than those of pacemakers and the technology used is more complicated they tend to pose greater conflicts, electromagnetic and mechanical forces (10 times higher than pacemaker), under MRI conditions. Device Therapy in Refractory Heart Failure

Advanced heart failure (HF) has been defined as persistent symptoms that limit daily life despite optical medical therapy, corresponding to New York Heart Association (NYHA) class III/IV symptoms or to the newer classification of stage D HF (ACC/AHA staging classification). It affects 10% of the HF population and is associated with a poor quality of life, recurrent hospitalizations and a mortality approaching 50% at 1 year and 80% at 5 years. Despite the widespread use of angiotensin converting enzyme (ACE) inhibitors, beta adrenergic blocking agents and spironolactone which improve the prognosis in mild to moderate stages, HF remains a progressive disease leading to decompensation and demand of both inotropic agents (class III recommendation according to ACC/AHA guidelines, considered solely for palliation in patients with end-stage disease) and diuretics to treat hypotension, impaired renal function and pulmonary congestion.
Heart transplantation (HT) is associated with nearly 90% 1-year survival, 60% 10-year survival and 95% freedom from symptoms and activity limitations in survivors throughout the follow-up period. Nevertheless, the lack of available donors and the large number of patients, who do not meet the criteria for transplantation, have spurred interest in cardiac resynchronization therapy (CRT) and mechanical circulatory support (MCS), providing alternatives for patients waiting for HT (bridge to transplantation, BTT), patients who are ineligible for HT (destination therapy, DT) or patients who are anticipated to recover after left ventricular unloading (bridge to recovery, BTR)... New Agents in the Treatment of Pulmonary Hypertension

Pulmonary Hypertension (PH) is defined as an increase in mean pulmonary arterial pressure (PAP) ≥25 mmHg at rest as assessed by right heart catheterization (RHC). As stated in the European Society of Cardiology (ESC) 2009 guidelines, it can be classified into 5 main categories, presented in Table 1. Pulmonary arterial hypertension is a rare disease with a prevalence of 15 cases /million adult population and an incidence of 2.4 cases/million adult population/year. The pathophysiology of PH is complex and it has not been elucidated in detail since it involves various biochemical pathways and cell types. Vasoconstriction, remodeling with extensive proliferation of the vessel wall cells causing progressive obstruction, inflammation and thrombosis have been implicated. Structural or functional abnormalities of potassium channels in the smooth muscle cells and endothelial dysfunction characterized by impaired nitric oxide (NO) and prostacyclin production or increased expression of thromboxane A2 and endothelin-1are thought to underlie the cellular changes.
Although groups two and three account for the majority of the cases, the trials testing and validating therapeutic options involve mostly patients with group one PH (Pulmonary Arterial Hypertension-PAH). Apart from conventional therapy including diuretics, oral anticoagulants, calcium channel blockers in case of established vasoreactivity, digoxin, oxygen administration and exercise –rehabilitation programs, specific drugs have emerged and gradually find their place in the management of PAH during the last decade. Hypertension in the Elderly

In our aging society, most of the elderly aged >65 years are affected by systolic hypertension (HTN) [blood pressure (BP) >140 mmHg], which constitutes a major risk factor for organ damage and cardiovascular (CV) events. Management of HTN in the elderly represents a therapeutic dilemma because HTN trials had upper age limits or did not present age-specific results. However, the HYVET trial documented that therapy is beneficial even in those >80 years. In the elderly, systolic BP and pulse pressure emerge as potent risk factors for CV events. In the past an empiric formula "100+age" was used to estimate appropriate systolic BP. Diastolic BP is more important in younger people <50 years.
Hypertension in the elderly is due to increased stiffness and pulse wave velocity of the great arteries with earlier return of reflected waves, causing high systolic BP, low diastolic BP, increased myocardial oxygen demand with higher peripheral resistance and limited organ perfusion. Furthermore, decreased renal function contributes to HTN through volume expansion, increased intracellular sodium, reduced Na-Ca exchange; K+ excretion is limited and plasma aldosterone is low, so that the elderly are prone to drug-induced hyperkalemia. Autonomic dysfunction and venous insufficiency contribute to orthostatic hypotension, resulting in falls, syncope, CV events or orthostatic hypotension. Secondary HTN should also be considered, including renal artery stenosis, obstructive sleep apnea, primary aldosteronism, hyper- or hypo-thyroidism, tobacco, alcoholism, caffeine, use of no-steroidal anti-inflammatory drugs, glucocorticoids, sex hormones... Athlete`s Heart or Athletic Heart Syndrome is a benign entity characterized by morphological changes of the heart muscle as a result of long-term adaptation to exercise. The hallmark of these alterations is a substantial enlargement of the heart due to both heart chamber dilatation and heart muscle mass hypertrophy. Although athlete’s heart has long been considered as a benign condition, there are data suggesting an association with sudden cardiac death (SCD) which can occur in athletes during exercise or even during daily activities. Indeed, hypertrophic cardiomyopathy (HCM), also characterized by marked cardiac hypertrophy, is the most common cause of SCD in young athletes. The association between HCM and athlete’s heart has been a matter of extensive discussion and debate among clinicians since an eccentric biventricular hypertrophy is almost universally present in athletes with a left ventricular diastolic volume >58 mm. This conflict has not yet been resolved, but, we have nowadays evidence that the ECG, although commonly altered by athletic cardiac remodeling, can assist in selecting athletes who are at substantial risk for an episode of SCD... Long-Term Results of Atrial Fibrillation Ablation

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia in the general population, affecting about 0.4% of the general population. Its prevalence increases with age reaching 15% in adults over 70 years of age. During the past decade, as techniques and technologies have improved, catheter ablation of AF has become a standard and effective therapy for patients with symptomatic and drug-refractory AF. The improved three-dimensional electroanatomic mapping systems and the induction in the clinical practice of other ablation techniques, such as cryoablation, have contributed to the worldwide increase of the number of ablation procedures. Catheter ablation seems to be superior to antiarrhythmic drug therapy (ADT) which is also associated with potential toxic or proarrhythmic effects after long term use. The recently presented data from RAAFT 2 study, showed that 55% of the patients who had randomized to AF ablation had had a recurrence compared to 72%, of those who had received ADT after 2 years follow up. For the first time, the 2012 updated guidelines from the European Society of Cardiology, recommend catheter ablation as the first line therapy in selected patients with paroxysmal AF alternative to ADT (class IIa, level B). The main target of the AF catheter ablation is the circumferential electrical isolation of the pulmonary veins (PVs) ostium or antrum. Development of Transcatheter Aortic Valve Implantation and its Clinical Implications

In the early 1990s the concept of transcatheter aortic valve implantation (TAVI) appeared challenging and totally unrealistic. It was a true “resurrection” for Cribier and his whole team performing the first TAVI in an inoperable patient in 2002, using a transeptal antegrade approach and balloon-expandable aortic valve prosthesis. Since then TAVI has been performed in more than 50000 patients worldwide. TAVI is currently indicated in patients with severe symptomatic aortic stenosis (AS) and acceptable life expectancy who are not suitable for aortic valve replacement (AVR) (indication class IB) or as an alternative to aortic valve replacement (AVR) in selected high-risk operable patients (class IIB), according to the “Heart Team” assessment.
The TAVI Heart Team comprised of clinical cardiologists, interventionalists, surgeons, anaesthetists and imaging specialists with expertise in the treatment of valve disease, selects patients suitable for TAVI taking into account advantages and disadvantages of both AVR and TAVI. A logistic EuroSCORE ≥20% (logistic EuroSCORE I tends to overestimate observed mortality risk by a factor of 2 to 3 and a newly updated logistic EuroSCORE II is currently available in clinical practice) or a Society of Thoracic Surgeons (STS) score >10% are suggested as indications for TAVI therapy. Recent publications have identified a number of baseline variables independently associated with mortality or poor outcome in patients undergoing TAVI (low body mass, functional status, left ventricular dysfunction, NT-proBNP, prior stroke, diabetes, chronic kidney disease, anemia, severe tricuspid and mitral regurgitation, porcelain aorta or history of chest radiation) which could be integrated into new scoring systems to quantify and predict the prognosis of TAVI both in the immediate and in the long term... Evaluation of Left Ventricular Diastolic Function by Echocardiography

Despite the knowledge that the heart spends almost two-thirds of its time in diastole (relaxing and filling), its contractile activity was for a long time considered the core of its mechanical function and over which major concerns had been focused. It was after the 1980s that the scientific community began to realize the clinical significance of diastolic dysfunction among patients with signs and symptoms of heart failure (HF) but in whom ejection fraction was rather preserved. The recognition of the latter condition as “heart failure with preserved ejection fraction” (HF-PEF) impelled major efforts in order to identify the pathophysiological mechanisms underlying this emerging concept. The impact of diastolic dysfunction on cardiac morbidity and mortality is becoming increasingly understood.
The hallmark of diastolic dysfunction is the impaired capacity to fill or maintain stroke volume without a compensatoy increase in filling pressures. Historically, invasive hemodynamics have provided useful information with respect to diastolic filling pressures, e.g., left atrial (LA) pressure and left ventricular (LV) end-diastolic pressure; left ventricular relaxation (time constant of relaxation-dP/dt) and operant chamber stiffness (pressure-volume loops, diastolic pressure contour). However, advances in echocardiographic assessment of LV diastolic function can lead to the replacement of invasive hemodynamics in the vast majority of patients. In the crucial question “can echocardiography accurately measure diastolic function?”, there are reasoned arguments on either side of the debate. With advancing age, all organ systems undergo anatomical and functional changes. Vasculature suffers generalized stiffening as a hallmark of this process. Although this phenomenon has been described long ago, only in recent years has the clinical significance of the stiffening of the large arteries been widely understood and its correlation with hypertension, coronary heart disease, stroke, heart failure and atrial fibrillation has been recognized. It has also been accepted that it mediates the vascular effects of diabetes mellitus, atherosclerosis and renal disease. 1-5
The arterial systems consists of two functional components with different structural characteristics: a) the large elastic arteries (aorta, carotid and iliac arteries) which store part of the blood ejected during systole and expel it to the periphery during diastole to provide the tissues with a relatively steady flow through the entire cardiac cycle and b) the muscular arteries (arteries below the axillary and femoral ones) which regulate the vascular tone and hence determine the peripheral resistance. The former have a thick tunica media in which the elastic fibers dominate and form numerous concentric layers. The later possess a tunica media characterized mainly by smooth muscle cells and the elastic component is confined to the thin internal and external elastic laminae. The different mechanical properties of the arterial tree along its course and the varying diameter of the arterial branches, gives rise to the reflected waves which are produced when the pulse wave encounters sites with different impedance. A part of these secondary waves amplifies the forward moving wave and another part moves backward to enhance the pressure of the central aorta. Normally it reaches its target in late systole or early diastole and it contributes to the diastolic flow towards the coronary arteries and the periphery... (excerpt)

Recent Data on Epicardial Ablation of Ventricular Tachycardia in Nonischemic Heart Disease

Epicardial ablation has been adopted during the last years, mainly as a supplementary technique after a failed endocardial ablation procedure, both in patients with ischemic and nonischemic ventricular tachycardias (VTs). Sosa and colleagues were the first who described the percutaneous subxiphoidal puncture to approach the epicardial space in 1996. Using the 3D electroanatomic mapping systems, the endocardial and epicardial substrate mapping have become feasible during the same procedure. Because of its complexity and its potential risks this process is performed only in high experienced centers by skilled operators with a large number of VT ablation procedures.
Endocardial ablation in patients with left ventricular nonischemic cardiomyopathy (NICM) has shown worst outcome compared with ablation in ischemic cardiomyopathy. The main reason seems to be the progressive nature of the disease and the presence of epicardial and intramural slow conduction areas forming reentry circuits. The pattern of fibrosis and scar in NICM is not predictable as in ischemic cardiomyopathy where it follows the distribution of the coronary artery disease. Data from the study of Hsia et al in patients with NICM and VT episodes supported that the critical endocardial low voltage substrate was located mainly in the basal and perivalvular area. In this study, epicardial mapping was performed only in few patients revealing abnormal fragmented potentials but data from later studies highlighted the important of epicardial substrate in NICM. Cano et al3 performed both endocardial and epicardial mapping in 22 patients either because of failed endocardial ablation or because of electrocardiographic signs suggesting epicardial localization of the exit point. Electroanatomic mapping revealed extended epicardial low voltage areas in the majority of the patients (about 82%) which were located mostly in basal left ventricular lateral wall. However, Haqqani et al described 31 of 266 patients with NICM (11.6%) who had septal involvement, mainly in the basal region, without lateral low voltage areas... (excerpt)

Iron Deficiency in Patients With Congestive Heart Failure

A large percentage of patients with chronic heart failure (HF) have anemia, defined as hemoglobin (Hb) of <12 g/dl, hovering around 30% in non-hospitalized HF patients and about 50% of hospitalized patients.1,2 The presence of anemia is an independent risk factor, associated with increased rates of mortality, HF hospitalization and morbidity. The cause of anemia in HF is multifactorial with 63.8% of patients having at least two factors that cause anemia. Chronic kidney insufficiency, iron deficiency, vitamin B12 deficiency, hemodilution, chronic diseases, and cachexia are the most common causes.3 Iron deficiency with or without anemia is common in patients with HF, relates to disease severity, and is a strong and independent predictor of outcome.4 Iron deficiency is defined as a ferritin level < 100μg/L or ferritin level 100-299 μg/L with a transferrin saturation < 20%.4 Anemic patients were more often iron deficient than non anemic patients. International clinical practice guidelines recommend early introduction and continued treatment with beta-blockers for all patients without contraindications after STEMI. Although there seemed to be little question that patients with STEMI, regardless of revascularization strategy, derive substantial benefits from both long- and short-term beta-blockade, there has been a paucity of high quality evidence supporting this notion and the majority of data predate modern reperfusion therapy and current medical management strategies with statins and antiplatelet agents. Recently published data question this “one-size-fits-all” approach, showing that the use of beta-blockers increased the risk of heart failure and cardiogenic shock with no mortality benefit.

Cardiac resynchronization therapy (CRT) is a relatively new therapeutic option for patients with systolic heart failure (HF) and electrocardiographic evidence of dyssynchrony. However, with current selection guidelines, still a proportion of patients do not respond to this interventional therapy. Several echocardiographic criteria have been proposed to address this issue, but research so far has failed to provide a single and simple measurement with adequate accuracy for CRT candidate selection. While investigation for this subject is still under way, new possible roles of echocardiography in CRT implementation arise, such as assistance in selecting the site of left ventricular (LV) pacing lead and optimizing CRT device programming during follow up visits.

Coronary angioplasty (or percutaneous coronary intervention, PCI) is a mature and widely diffused treatment technique for coronary artery disease. Over the last decades, great evolution has been realized concerning the related technology, the pharmacologic armamentarium and operators’ experience resulting in improved safety and success rates of PCI. Despite the fact that associated risks have declined over time, since PCIs are invasive procedures, complication rates have always been and still are a vexing reality. They concern the cardiologist who sets the indication, the interventionalist who performs the procedure but most importantly the patient who should be well informed for the anticipated benefits and risks before giving his written informed consent. A concise update on recent data about the most important issues regarding PCI complications is attempted herein.

To avoid use of a Swan-Ganz catheter and its attendant complications, new technologies have now become available to help the clinician perform a less invasive hemodynamic monitoring in patients in the intensive care unit (ICU). Among these, conventional echocardiography, the esophageal Doppler, pulse pressure analysis, the transpulmonary thermodilution, the indicator dilution and the thoracic electrical bioimpedance and bioreactance, all aim at measuring stroke volume and cardiac output by less invasive means and they are herein briefly reviewed.

Arrhythmogenic right ventricular cardiomyopathy/ dysplasia (ARVC/D), mostly affecting young/middle-aged individuals, poses a significant risk of malignant ventricular arrhythmias (VAs) and subsequent sudden cardiac death (SCD). Antiarrhythmic agents (AAA) provide insufficient arrhythmia suppression and prevention and can be proarrhythmic. Thus, the implantable cardioverter-defibrillator (ICD) is considered the first-line treatment, especially in patients with secondary prevention indication. Nevertheless, catheter ablation is an additional therapy to the ICD which has proved its efficacy in primary and secondary prevention of fatal arrhythmias and sudden cardiac death. The superiority of the combined endo- and epicardial VT ablation in this population is clear since ARVC/D substrate has been shown to be mostly epicardial. Due to progressive nature of ARVC/D, ablation seems to be a useful tool for the patients who experience recurrent VT episodes or electrical storms.

Hypertrophic cardiomyopathy (HCM), in contrast to common prejudications, has been recognized as the most common genetically determined cardiac disease. It is inherited with an autosomal dominant pattern and many different mutations in numerous genes responsible for the production of sarcomere proteins and other regulatory molecules contributing to the systolic function of cardiac myocyte have been identified. The natural course of the disease is benign in the majority of cases and most patients achieve near-normal life expectancy. Unfavorable prognosis is established mainly on the basis of clinical, morphological and family history data. The genotype contributes undoubtedly to the clinical picture. However, the existing data do not allow for a consistent genotype-phenotype correlation and hence genetic information is not incorporated in modern risk assessment clinical tools. In this brief review we summarize the recent data on the genetic characterization of HCM.

The Current Role of Glycoprotein IIb/IIIa Inhibitors in Percutaneous Coronary Intervention

The central role of platelets in acute coronary syndromes (ACS) and percutaneous coronary interventions (PCI) is well appreciated. The various platelet activation mechanisms finally lead to expression and activation of surface glycoprotein IIb/IIIa receptors that mediate platelet aggregation and thrombus formation. Glycoprotein IIb/IIIa inhibitors (GPIs) are the most potent antiplatelent agents and their role in ACS treatment and PCI has been dominant in the recent past. The advent of stents and thienopyridines minimized ischemic complications and in parallel the role of GPIs in low risk PCI. Despite being effective in decreasing PCI-related ischemic complications, the major drawback of GPI use is a relative increase of bleeding that can adversely affect prognosis. The availability of bivalirudin, which is regarded as an equally effective but safer antithrombotic agent when compared to the combination of heparin and GPIs, despite an ongoing controversy, has also led to a decrease of GPI use in PCI for ACS. Finally the advent of novel potent antiplatelet agents (prasugrel, ticagrelor and soon cangrelor) further contained GPI use in patients with ischemic – thrombotic risk that clearly exceeds bleeding risk and mainly for bail-out in case of a thrombotic event during PCI. A concise overview of accumulated data regarding optimal use of GPIs as determined by large clinical trials and recent guidelines is herein attempted.

Current Data on the Role of Specific Antidotes for the Reversal of Non-Vitamin K Oral Anticoagulant Action

The increasing use of the non-vitamin K oral anticoagulants during the recent years was associated with the need of development of targeted agents that could reverse the anticoagulative activity in cases of severe bleeding episodes or urgent high risk operations. Thus, several reversal agents are currently in development and the early results seem promising. Idarucizumab is a monoclonal antibody that can immediately and specifically reverse dabigatran action. Andexanet alfa is a recombinant modified factor Xa that can bind and reverse factor Xa inhibitors, including rivaroxaban, apixaban and edoxaban, and low molecular weight heparin. Aripazine is a universal reversal agent small molecule that can reverse the action of factor Xa inhibitors, unfractionated and low molecular weight heparin and possibly dabigatran. Currently, only idarucizumab has received approval from the United States Food and drug Administration for the reversal of the dabigatran. Rhythmos 2016;11(3):70-72.

Takotsubo Syndrome: A Brief Review in Light of New Evidence

“Takotsubo Syndrome” (TTS) is now recognized as a distinct entity within the spectrum of stress-induced cardiomyopathies. It is believed that enhanced sympathetic stimulation induces transient myocardial stunning through a variety of mechanisms that include epicardial spasm, ischemia due to microvascular dysfunction, and direct cardiomyocyte toxicity from catecholamine-mediated calcium overload. Once associated with emotional or significant physical stress, it is now clear that even minor stressors can trigger the syndrome and that roughly 30% of patients with TTS have no identifiable trigger at all. Traditionally regarded benign, TTS is now recognized as a potentially fatal condition with serious complications and considerable short-term mortality, similar to that of acute coronary syndromes. The major features of the TTS regarding clinical characteristics, pathogenesis and treatment options are herein briefly reviewed. Rhythmos 2016;11(4):90-95.

Inappropriate sinus tachycardia (IST) is a syndrome of cardiac and extracardiac symptoms characterized by rapid sinus heart rate at rest (>100 bpm) or with minimal activity and disproportionate to the physiologic demands. Patients with this unique and puzzling arrhythmia may require restriction from physical activity. The responsible mechanisms for IST are not completely understood. IST and postural orthostatic tachycardia syndrome (POTS) are the 2 sides of the same coin. It is important to distinguish IST from so-called appropriate sinus tachycardia and from POTS, with which an overlap may occur. As the long-term outcome seems to be benign, treatment may be unnecessary, or may be as simple as physical training. However, for patients with intolerable symptoms, therapeutic measures are warranted. Beta-adrenergic blockers, considered a first-line therapy, are usually ineffective even at high doses; the same applies for most other medical therapies. Ivabradine seems to be more effective than beta-blockers especially in the non- hypertensive patients. In rare instances, catheter- or surgically- based right atrial or sinus node modification may be helpful, but even this is fraught with limited efficacy and potential complications. Overtreatment, in an attempt to reduce symptoms, can be difficult to avoid, but is discouraged. In this report, we will be review IST, explore its mechanisms and evaluate possible management strategies.

Patients with diabetes mellitus commonly are suffering from atrial fibrillation since these two conditions share pathophysiological mechanisms. Catheter ablation of atrial fibrillation is the cornerstone therapy for rhythm control in symptomatic patients with paroxysmal or persistent atrial fibrillation. Data regarding the efficacy and safety of catheter ablation from large randomized clinical trials in this population are limited. The only available data from small clinical trials and meta-analyses have shown a superiority of catheter ablation compared to antiarrhythmic drugs although there are some limitations. With the evolution in cardiovascular disease understanding and the application of advanced interventional therapies, antithrombotic medication has become the cornerstone of the medical management of cardiovascular patients. However, as older and new trials have confirmed, these drugs carry a substantial risk for hemorrhagic complications, especially from the gastrointestinal (GI) tract, which are accompanied by a significant mortality risk. In case of such an event, the clinician must decide whether to discontinue or not the medication and for how long, and he is called to balance the potential risk of thrombosis and recurrent bleeding. In this brief review, we present the studies which address this issue in order to elicit practical conclusions. (Rhythmos 2017;12(3): 45-49).

Is Atherosclerosis Amenable to Anti-Inflammatory Compounds?

The hypothesis of an inflammatory component in the pathogenesis of atherosclerosis has been under investigation for a long time but the data have not been conclusive. No clinical trial of anti-inflammatory, anti-oxidant, or anti-bacterial agents has ever proven efficacious. However, the recent announcement and publication of the CANTOS trial has raised expectations. This pivotal trial showed for the first time that anti-inflammatory therapy targeting the interleukin-1β innate immunity pathway with subcutaneous injections of canakinumab every 3 months conferred significantly, albeit modestly (15%), lower rate of the composite end-point of nonfatal MI, nonfatal stroke and cardiovascular (CV) death than placebo, independent of lipid-level lowering, in patients with prior myocardial infarction (MI). An unexpected corollary of this investigation revealed significant concomitant reduction in lung cancer mortality. Despite the initial enthusiasm about the trial results, sceptics point to that fact that CV mortality was not affected, while the incidence of fatal infections was much higher with the drug compared with placebo, and the cost of this therapy remains currently prohibitive for wider use. Ongoing and future trials with similar or more convenient and less expensive anti-inflammatory agents may provide more data whether such results are reproducible and/or supportive or evidential of the inflammatory hypothesis of atherosclerosis. Insights into the Clinical Spectrum of Catecholaminergic Polymorphic Ventricular Tachycardia

Catecholaminergic polymorphic ventricular tachycardia (CPVT) is induced by stress or exertion especially in young individuals with normal baseline ECG and without any structural heart disease. The most common type of ventricular tachycardia (VT) in these patients is bidirectional VT but could also be polymorphic VT or ventricular fibrillation. These two main types of CPVT are caused by mutations on the ryanodine (RyR2) or calsequestrin (CASQ2) receptor, with an autosomal dominant and recessive inheritance pattern respectively. The prognosis is dismal without treatment and the main therapeutic approach consists of administration of beta blocker, flecainide, calcium channel blockers or ICD implantation. Genetic testing is important for all family members of CPVT probands in order to identify asymptomatic carriers. Rhythmos 2018;13(1):6-8.

Cardiovascular disease is highly prevalent in modern western societies and aspirin has been established as an irreplaceable drug in such patients. A small but non-negligible number of these patients, manifest hypersensitivity to aspirin and /or other non-steroidal anti-inflammatory drugs (NSAIDs), which can take different forms depending on the underlying pathophysiology and clinical variability. In this brief communication, we review the types of hypersensitivity to NSAIDs, the diagnostic steps and its management, focusing on desensitization protocols in patients who require coronary interventions and long-term salicylate administration. The Contemporary Role of Percutaneous Coronary Intervention in Left Main Disease Management

Since the 1980s coronary artery bypass grafting (CABG) has been considered as the gold standard for treating left main disease. The continuous evolution of tools and techniques concerning coronary angioplasty and the advent of drug-eluting stents (DES) have established percutaneous coronary intervention (PCI) as a possible alternative to CABG. Initial randomized studies that were conducted during the previous decade, with main representative the SYNTAX study, compared conventional CABG to PCI with first generation DES and showed non-inferiority regarding hard clinical outcomes. The results of two randomized studies comparing CABG versus PCI with second generation DES were simultaneously published in the end of 2016 to further support PCI as a credible alternative to CABG for patients with left main disease with low and intermediate SYNTAX scores. A Heart Team is indispensable to best evaluate the anatomic parameters of coronary lesions, the clinical variables and the technical possibilities regarding disease complexity in order to define the ideal revascularization strategy for each patient in the elective setting. Percutaneous treatment for left main disease has been increasingly performed during the last decade, since the procedural steps and optimal techniques for left main PCI are nowadays standardized and well described in expert consensus documents and therefore should be respected and applied in order to optimize patient outcomes. Atrial fibrillation in patients with heart failure with reduced left ventricular ejection fraction is common and is associated with an increased risk of mortality, hospitalization and all-cause death. Rhythm control via medical treatment in these patients is restricted to use of amiodarone as the only suitable antiarrhythmic drug with the least proarrhythmic effect compared to other antiarrhythmic drugs for this population of patients. Over the last years there is a growing evidence that catheter ablation is beneficial in patients with heart failure since randomized clinical trials with hard endpoints have already been published. However, there are still unanswered questions about the patient categories that could benefit from the ablation procedures and the optimal ablation strategies in these patients. Cancer is accompanied by an increase of both thrombotic and hemorrhagic complications. Thus, the management of venous thromboembolism (VTE) or atrial fibrillation (AF) presents certain difficulties in oncologic patients. So far, low molecular weight heparins (LMWHs) have been proved the most effective and safe agents for long-term use in the VTE setting. However the novel oral anticoagulants (Direct Oral Anti Coagulants-DOACs), which are more friendly to use and begin to displace conventional anticoagulation in non-cancer patients, emerge as an attractive alternative. We present the latest data from randomized trials, meta-analyses of DOACs in oncologic populations and recent recommendation papers, as these agents claim their role in the management of cancer patients. The no-reflow phenomenon (NRp), a dreaded complication of primary and also of any complex percutaneous coronary intervention (PCI), is characterized by insufficient myocardial perfusion in a territory of a coronary artery without evidence of mechanical obstruction. Microvascular injury is the underlying mechanism of NRp and its manifestation is not only impaired TIMI flow (<3), but also impaired TIMI myocardial perfusion grade (TMPG) which should be assessed in case of chest pain, persistent ST segment elevation or hemodynamic compromise despite the presence of TIMI III flow. The NRp mechanism is multifaceted, the evidence base for its treatment is inconsistent and limited, but its predictors are well known. The armamentarium against NRp consists of preventive and therapeutic strategies, both mechanical and pharmacological. A brief overview of all the above issues concerning NRp is attempted herein. Premature ventricular contractions (PVCs) are common and are considered benign in the absence of structural heart disease. However, high burden of PVCs potentially on 24-hour Holter monitoring, can potentially cause left ventricular dysfunction. In this case, catheter ablation has been demonstrated to be effective at PVC suppression and is associated with improvement or normalization of ventricular function. Diabetes mellitus (DM) is nowadays considered an epidemic in the industrialized world. Among other complications, it affects the heart and is accompanied by an increased cardiovascular mortality. The development of heart failure (HF) in DM occurs independently of any coronary artery disease or hypertension, and this entity is termed “diabetic cardiomyopathy”. Cardiac hypertrophy, abnormal ventricular strain features, diastolic dysfunction and subsequently systolic impairment are the cardinal imaging characteristics. The pathophysiology of the disease is complex and multifactorial and, as a consequence, the therapeutic options, on top of the conventional HF medications, should aim at reversing the pathophysiological processes. Hereby we briefly review the manifestations of diabetic cardiomyopathy, describe the pathogenetic mechanisms and discuss the potential therapeutic targets. Rhythmos 2019;14(4):71-76.

Transcatheter aortic valve implantation (TAVI) was initially reserved for inoperable or high surgical risk patients with severe symptomatic aortic valve stenosis, but after the recent publication of randomized studies comparing TAVI to surgical aortic valve replacement (AVR) among intermediate risk (PARTNER 2 and SURTAVI) and low risk patients (PARTNER 3 and Evolut Low Risk), the momentum for further expansion of TAVI at the expense of AVR seems irreversible. The main obstacle before the wider application of TAVI for intermediate and lower risk patients is the uncertainty regarding bioprosthetic valve durability and the potential for structural deterioration and dysfunction over time. A concise overview regarding bioprosthetic valve durability issues, the relevant current scientific data and their importance for TAVI patient selection and management is herein presented. Atrial Fibrillation in Heart Failure With Preserved Ejection Fraction: Pathophysiology and the Role of Catheter Ablation

Atrial fibrillation (AF) and heart failure (HF) often coexist, and the prognosis of patients who have both these conditions is worse than those with either condition alone. Heart failure with preserved ejection fraction (HFpEF) is a clinical condition that was initially characterized as diastolic dysfunction HF, then as HF with normal ejection fraction, and more recently as HFpEF. About one-third of patients with HFpEF suffer from AF. Although, both clinical entities share common pathophysiologic mechanisms, current knowledge of the relationship between AF and HFpEF is limited. Catheter ablation, although data from randomized trials in this category of patients are limited, seems to have beneficial effects regarding maintenance of sinus rhythm and re-hospitalization rates. Premature ventricular complexes (PVCs) are a common feature specially at increasing age. In the normal heart they are usually benign, but in some cases, they may be associated with sudden cardiac death or risk of heart failure. Although the pathophysiological mechanisms of these effects are not yet fully clarified, PVCs’ potential to cause a form of cardiomyopathy is now widely accepted. In this brief review we present the main evidence regarding ventricular ectopy and its consequences, in individuals without pre-existing heart disease. Rhythmos 2020;15(3): 47-51.

Novel Angiography-Derived Techniques for Functional Assessment of Coronary Artery Disease

Background: The importance of coronary physiology in identifying hemodynamically significant lesions is well established.
Objective: The current review summarizes the most important studies of the novel techniques developed over the last decade that allow the computation of fractional flow reserve (FFR) from either invasive or computed coronary angiography.
Methods: A systematic review of all published research in PubMed and Google Scholar databases, regarding the angiography-derived functional assessment of coronary artery disease (CAD) has been performed. The following terms were used: “functional angiography”, “quantitative flow ratio”, “computed tomography-derived FFR”, “FFR angiography” and “virtual FFR”.
Results: Several multicenter clinical trials have presented the currently available techniques for physiological assessment of coronary artery stenosis, such as quantitative flow ratio (QFR), computed tomography-derived FFR, FFRangio and virtual FFR, their theoretical basis and methodology, as well as their diagnostic performance, using invasive FFR as reference standard.
Conclusion: A variety of novel angiography-derived techniques for physiological assessment of CAD exist, showing high diagnostic performance and are expected to increase the use of coronary physiology in the guidance of clinical decision making upon revascularization strategy. Ventricular Arrhythmias Originating from the Papillary Muscles of the Left Ventricle in the Structurally Normal Heart and the Role of Catheter Ablation

Ventricular arrhythmias arising from the left ventricular papillary muscles, having a right bundle branch block (RBBB) QRS morphology on the electrocardiogram, account for approximately 15% of arrhythmias in a structurally normal heart and have usually a benign prognosis. The mitral valve usually has two papillary muscles, anterolateral and posteromedial, with the latter one being always more arrhythmogenic. These arrhythmias are not typically inducible by programmed ventricular or atrial stimulation, suggesting a non-reentrant mechanism. Ventricular arrhythmias originated from the papillary muscles should be distinguished from other idiopathic left ventricular arrhythmias such as fascicular or mitral annular arrhythmias, which also have a RBBB pattern. Catheter ablation in these cases is always challenging and the recurrence risk is higher compared to other idiopathic ventricular arrhythmias. Rhythmos 2021;16(1):7-10.

Obesity is a relatively common characteristic in modern western world populations and has a significant impact on the person’s health status. Despite the fact that it constitutes a well-described risk factor for thromboembolic events, the optimal anticoagulation strategy in obese patients remains vague. The available data suggest that while standard fixed enoxaparin doses (for prophylactic purposes) can lead to subtherapeutic effect, the conventional weight-based dosing schemes may result in overtreatment. Although not particularly strong, contemporary evidence indicate that a dose reduction in morbidly obese patients will likely result in a therapeutic anti-Xa level without an increased probability for bleeding or VTE. During the last two decades transcatheter aortic valve implantation (TAVI) and mitral valve transcatheter edge-to-edge repair (TEER) have evolved in parallel to provide percutaneous interventional treatment solutions for patients with the two most common valvular heart diseases, aortic stenosis and mitral regurgitation, respectively. Having initially been conceived and used to treat patients considered inoperable, TAVI indications have been expanded to include high operative risk and more recently moderate or even low risk patients. The TAVI techniques have also improved to permit tackling challenging complex anatomies and bioprosthetic valve degeneration and structural failure. Similarly, TEER has been initially used to treat inoperable patients with severe symptomatic primary mitral regurgitation (MR), but recently published data have shown that it can provide benefit in carefully selected patients with refractory to optimal medical treatment (OMT) symptomatic severe secondary MR. Furthermore, apart from TEER with the traditional MitraClip system and its iterations, a second TEER system (Pascal) has provided promising results in initial clinical trials and can alternatively be used. Long QT Syndrome in Athletes: Challenges in the Diagnosis and Management

The long QT syndrome (LQTS) is an inherited channelopathy characterized by a prolonged QT interval without structural heart disease posing a risk of sudden cardiac death. Especially in athletes, diagnosis is sometimes more difficult due to bradycardia and to the QT prolongation induced by the exercise. Based on limited clinical data which tend to demonstrate that exercise, especially swimming, is a trigger for cardiac events, participation in any competitive sports practice is not supported by 2020 European guidelines. However, based on retrospective studies and involving the patient-athlete in shared decision making, the 2015 US guidelines are more lenient. Rhythmos 2021;16(4): 79-81.

Endocardial Left Atrial Appendage Occlusion and its Place in Contemporary Management of Atrial Fibrillation

Atrial fibrillation (AF) is the most common arrhythmia, affecting 1-2% of the general population in Western countries and it is a well-known risk factor for cardioembolic ischemic stroke, which increases the risk 4 to 5 times. Stroke is responsible for 14% of all annual deaths and every year 8 million people suffer a stroke in European countries. The standard stroke preventive strategy in patients with AF is the administration of anticoagulants. However, contraindications to these agents are not rare, the use of anticoagulants, especially the vitamin-K antagonists, is quantitatively sub-optimal, and many patients with high thromboembolic risk also have increased bleeding risk. Considering that the most frequent location of intracardiac thrombi associated with AF is the left atrial appendage (LAA), the endocardial occlusion of this structure has emerged as a potential alternative treatment. The Aftermath of a Negative Third FAME

The current guideline recommendations regarding myocardial revascularization of patients with multivessel coronary artery disease (CAD) mostly advocate coronary artery bypass grafting (CABG) over percutaneous coronary intervention (PCI), especially for patients with diabetes. However, in certain clinical cases, PCI can be considered. FAME and FAME 2 studies had demonstrated the superiority of fractional flow reserve (FFR)-guided PCI over angiography-guided PCI and over optimal medical therapy (OMT) alone respectively. FAME 3 study (Fractional Flow Reserve versus Angiography for Multivessel Evaluation), published early in 2022, was a study that investigated how PCI guided by FFR measurements can perform compared to CABG guided mostly by coronary angiography for the revascularization of patients with three-vessel CAD. Stable patients with an average SYNTAX score of 26 were randomized, while patients with left main disease, recent ST segment elevation myocardial infarction (STEMI), left ventricular ejection fraction <30% or cardiogenic shock were excluded. Regarding the composite primary endpoint of major adverse cardiovascular events (MACE), including death, non-fatal myocardial infarction, stroke or repeat revascularization, at 1 year follow-up, FFR-guided PCI failed to be proven as non-inferior compared to CABG. The findings of FAME 3 as added to those of FAME and FAME 2 should be considered in the context of current guidelines for myocardial revascularization and do not seem practice changing. Coexistence of aortic valve stenosis (AS) and cardiac amyloidosis (CA) is frequent in the elderly as both conditions increase with age. Wild type transthyretin (TTR) amyloidosis is the most common type of amyloidosis in AS cohorts. Cardiac amyloidosis and AS share common clinical characteristics making diagnosis of dual pathology challenging. However, certain features should raise suspicion of CA presence in AS patients leading to specific imaging modalities to confirm diagnosis. Dual pathology (AS & CA) increase mortality risk and prompt diagnosis is crucial. Novel pharmacological agents targeted for TTR CA should be initiated to improve prognosis. The role of the Heart Team for decision-making in these cases regarding the optimal management of AS is crucial. Transcatheter aortic valve replacement may be the preferred procedure in these high-risk patients. Rhythmos2022;17(3): 53-57.

Pulsed Field Ablation (PFA) for Pulmonary Vein Isolation in τhe Treatment οf Atrial Fibrillation

Pulsed‐field ablation (PFA) is a promising new ablation modality for the treatment of atrial fibrillation (AF). Pulmonary vein isolation (PVI) remains the most frequently used treatment strategy of any AF ablation procedure. A major limitation of current catheter ablation procedures is PVI reconnection, reported to be about 20% of patients. Pulsed field ablation is a novel energy source by which high-voltage electric pulses are used to create pores in the cellular membrane (i.e., electroporation), leading to cellular death. The amount of energy required to produce irreversible electroporation is highly tissue dependent reducing the risk of damage to surrounding non-cardiac tissues. The tissue specificity of PFA may result in a wide therapeutic range and improved safety profile during atrial fibrillation ablation. There has been no evidence of incidental phrenic nerve injury, and finally, PFA has been shown not to injure esophageal tissue when directly applied to the esophagus or indirectly through ablation in the left atrium.
